摘　　要：目的:探讨入院时超声参数收缩期流速(Systolic flow velocity,VS)、舒张期流速(Diastolic flow velocity,VD)、阻力系数(Resistance index,RI)与血清孕酮水平联合检测对异位妊娠患者进行保守治疗疗效的预测价值。方法:选取2022年8月~2023年4月周口市第六人民医院收治的63例接受保守治疗的异位妊娠患者作为研究对象,根据治疗结局分为有效组(n=47)和无效组(n=16)。比较入院时超声相关参数VS、VD、RI值及血清孕酮水平,分析入院时超声相关参数VS、VD、RI值及血清孕酮水平与异位妊娠患者进行保守治疗疗效的相关性;分析超声相关参数VS、VD、RI值及血清孕酮水平联合预检测对异位妊娠患者进行保守治疗疗效的预测价值;分析入院时超声相关参数VS、VD、RI值及血清孕酮不同水平患者治疗无效的危险度。结果:有效组入院时超声相关参数VS、VD、RI值及血清孕酮水平低于无效组(P<0.05);入院时超声相关参数VS、VD、RI值及血清孕酮与异位妊娠患者保守治疗无效呈正相关(P<0.05);入院时超声相关参数VS、VD、RI值及血清孕酮水平联合检测预测异位妊娠患者进行保守治疗无效的曲线下面积(Area under curve,AUC)为0.873,高于上述指标单独检测;入院时超声相关参数VS、VD、RI值及血清孕酮高水平异位妊娠患者治疗无效的危险度是低水平的3.131、5.271、2.708、3.826倍。结论:超声相关参数VS、VD、RI值及血清孕酮水平与异位妊娠患者进行保守治疗效果具有密切关系,各指标联合检测对异位妊娠患者进行保守治疗效果具有较高的预测价值,可为临床早期预测异位妊娠患者进行保守治疗结局提供参考依据。Objective:To investigate the predictive value of ultrasonic parameters of systolic flow velocity(VS),diastolic flow velocity(VD),resistance index(RI)and serum progesterone level combined detection at admission for conservative treatment of ectopic pregnancy.Methods:A total of 63 patients with ectopic pregnancy who received conservative treatment in Zhoukou Sixth People's Hospital from August 2022 to April 2023 were collected.According to the treatment outcome,they were divided into an effective group(47 cases)and an ineffective group(16 cases).The values of VS,VD,RI and serum progesterone level at admission were compared.The correlation of VS,VD,RI and serum progesterone level with conservative treatment of ectopic pregnancy was analyzed.The prognostic value of VS,VD,RI and serum progesterone level combined detection for conservative treatment of ectopic pregnancy was analyzed.The risk of treatment failure in patients with VS,VD,RI and different serum progesterone level were analyzed.Results:The values of VS,VD,RI and serum progesterone level in the effective group were lower than those in the ineffective group(P<0.05).At admission,the values of VS,VD,RI and serum progesterone level were positively correlated with the ineffective conservative treatment of ectopic pregnancy(P<0.05).The area under curve(AUC)of the combined detection of VS,VD,RI and serum progesterone level for predicting the failure of conservative treatment in ectopic pregnancy patients was 0.873,which was higher than that of the above indicators alone.The risk of treatment failure in patients with ectopic pregnancy with high level of progesterone were 3.131,5.271,2.708,3.826 times higher than those with low level.Conclusion:Ultrasound related parameters VS,VD,RI and serum progesterone level are closely related to the effect of conservative treatment in patients with ectopic pregnancy.The combined detection of all indicators has a high predictive value for the effect of conservative treatment in patients with ectopic pregnancy,and can provide a re
