摘　　要：目的:探索甲泼尼龙钠不同给药方式治疗低频下降型突发性聋(Low-tone sudden deafness,LTSD)的效果。方法:选取我院2020年4月至2023年8月期间收治的106例LTSD患者,根据治疗方法不同分为滴注组和注射组,各53例分别应用甲泼尼龙琥珀酸钠静脉滴注和鼓室注射进行治疗。治疗前、治疗10 d后,采用眩晕障碍评定量表评估眩晕情况,耳鸣致残量表评估耳鸣情况,眩晕、耳鸣等症状及听力恢复情况评估临床效果,纯音电测听仪测定0.5、2、4 kHz频率的听阈状况,SA-5600型全自动血流变测试仪测定全血低切还原黏度、全血高切还原黏度、血浆黏度,全自动血液分析仪测定血小板计数,循环酶法测定同型半胱氨酸。结果:治疗10 d后,注射组临床总有效率高于滴注组;眩晕障碍评定量表、耳鸣致残量表评分与听力阈值及全血低切与高切还原黏度、血浆黏度、血小板计数、同型半胱氨酸水平低于滴注组(P<0.05)。结论:鼓室注射甲泼尼龙琥珀酸钠可改善LTSD患者血流动力学,调节血液指标与听力阈值,改善眩晕耳鸣情况及临床效果。Objective:To explore the effect of different administration modes of methylprednisolone sodium on Low-tone sudden deafness(LTSD).Methods:A total of 106 patients with LTSD admitted to our hospital from April 2020 to August 2023 were selected and divided into an infusion group and an injection group based on different treatment methods.Fifty-three patients in each group were treated with methylprednisolone sodium succinate intravenous infusion and tympanic injection,respectively.Before treatment and 10 days after treatment,the vertigo assessment scale was used to assess the condition of vertigo,the tinnitus disability scale was used to assess the condition of tinnitus,the symptoms of vertigo,tinnitus,and hearing recovery were evaluated for clinical efficacy.The pure-tone electric audiometer was used to measure the hearing threshold at frequencies of 0.5,2,and 4 kHz.The SA-5600 fully automatic hemorheological tester was used to measure the whole blood low shear reduction viscosity,whole blood high shear reduction viscosity,and plasma viscosity.The fully automatic blood analyzer was used to measure platelet count,and the circulating enzyme method was used to measure homocysteine.Results:After 10 days of treatment,the total clinical effective rate of the injection group was higher than that of the infusion group;The vertigo assessment scale,tinnitus disability scale scores,hearing thresholds,whole blood low shear and high shear reduction viscosity,plasma viscosity,platelet count,and homocysteine levels were lower in the infusion group than in the infusion group(P<0.05).Conclusion:Intratympanic injection of methylprednisolone sodium succinate can improve hemodynamics,regulate blood indicators and hearing thresholds,improve dizziness and tinnitus in patients with LTSD,and its clinical effects.
