超声引导下真空辅助微创旋切与空芯针穿刺活检用于乳腺BI RADS 3、4类病灶的诊断价值

摘  要:目的:探讨超声引导下真空辅助微创旋切与空芯针穿刺活检用于乳腺影像学报告和诊断系统(Breast Imaging Reporting and Data System,BI-RADS)分级3、4类病灶的诊断价值。方法:回顾性分析2021年8月至2023年4月期间于我院收治的72例乳腺病变患者临床资料,经B超等影像学检查以及乳腺分级标准BI-RADS分级确认为乳腺BI RADS 3、4类病灶,根据所行活检方式不同分为微创旋切组(行超声引导下真空辅助微创旋切)38例、空芯针穿刺组(行超声引导下空芯针穿刺)34例,以术后病理组织学结果为金标准,展开Kappa一致性分析,比较两种活检方式的诊断价值,计算敏感度、特异性等相关参数,统计两组活检相关并发症发生情况。结果:微创旋切组检出恶性病变15例(39.47%),良性病变23例(60.53%),空芯针穿刺组检出恶性病变12例(35.29%),良性病变22例(64.71%);与术后病理结果比较,微创旋切组Kappa值0.840,空芯针穿刺组Kappa值0.282,经一致性分析,微创旋切组灵敏度、特异度、准确率、阳性诊断值均高于空芯针穿刺组(P<0.05)。结论:超声引导下真空辅助微创旋切、空芯针穿刺活检对乳腺BI-RADS分级3、4类病灶均具有一定诊断价值,但真空辅助微创旋切诊断价值相对更高。Objective:To investigate the diagnostic value of ultrasound-guided vacuum-assisted minimally invasive rotary cutting and core needle biopsy for Breast Imaging Reporting and Data System(BI-RADS)3 and 4 breast lesions.Methods:A retrospective analysis was conducted on the clinical data of 72 patients with breast lesions admitted to the hospital from August 2021 to April 2023.The lesions were confirmed as BI-RAD 3 or 4 by imaging examinations such as B-ultrasound and BI-RADS.According to different biopsy methods,the patients were divided into the minimally invasive rotary cutting group(ultrasound-guided vacuum-assisted minimally invasive rotary cutting,38 cases)and the core needle biopsy group(ultrasound-guided core needle biopsy,34 cases).With postoperative histopathological results as the gold standard,Kappa consistency analysis was conducted.Diagnostic values of the two biopsy methods were compared.Relevant parameters such as sensitivity and specificity were calculated.The incidence of biopsy related complications in the two groups was statistically analyzed.Results:15(39.47%)cases with malignant lesions and 23(60.53%)cases with benign lesions were detected in the minimally invasive rotary cutting group.12(35.29%)cases with malignant lesions and 22(64.71%)cases with benign lesions were detected in the core needle biopsy group.Compared with postoperative pathological results,Kappa values of minimally invasive rotary cutting and core needle biopsy were 0.840 and 0.282,respectively.The sensitivity,specificity,accuracy and positive predictive value of minimally invasive rotary cutting were higher than those of core needle biopsy group(P<0.05).Conclusion:Both ultrasound-guided vacuum-assisted minimally invasive rotary cutting and core needle biopsy are helpful for diagnosing BI-RADS 3 and 4 breast lesions,but the predictive value of ultrasound-guided vacuum-assisted minimally invasive rotary cutting is relatively higher.
