多节段脊柱融合术在治疗复杂脊柱退行性疾病中的疗效及邻近节段退变情况观察

摘　　要：目的:探索多节段脊柱融合术(MFSS)在治疗复杂脊柱退行性疾病中的疗效。方法:选取2021年2月至2023年2月期间在本院接受治疗复杂脊柱退行性疾病患者111例作为研究对象。随机将患者分为对照组(55例)和观察组(56例)。对照组采用传统手术;观察组采用MFSS。分析比较两组的临床疗效、疼痛程度(VSA评分)、脊柱功能(ODI指数)、生活质量、邻近节段退变情况及并发症发生率。结果:对照组的脊柱融合率显著低于观察组(P<0.05)。术后6m,两组的VSA评分与ODI指数均较术前有下降,且观察组的VSA评分与ODI指数均显著低于对照组(P<0.05)。术后,两组的生活质量评分均比术前提高,且观察组的生活质量评分显著高于对照组(P<0.05)。术后6m,观察组的邻近节段退变发生率显著低于对照组(P<0.05)。术后,观察组的并发症发生率显著低于对照组(P<0.05)。结论:多节段脊柱融合术应用在复杂脊柱退行性疾病的治疗中,能提高疗效,降低并发症发生率,减缓邻近节段的退变,改善脊柱功能。Objective:To explore the efficacy of multilevel spinal fusion(MFSS)in the treatment of complex spinal degenerative diseases.Methods:111 patients with complex spinal degenerative diseases who received treatment in our hospital from February 2021 to February 2023 were selected as the study subjects.The patients were randomly divided into control group(55 cases)and observation group(56 cases).The control group received traditional operation.MFSS was adopted in the observation group.The clinical efficacy,pain degree(VSA score),spinal function(ODI index),quality of life,adjacent segment degeneration and complication rate of the two groups were analyzed and compared.Results:The spinal fusion rate in control group was significantly lower than that in observation group(P<0.05).6 months after operation,VSA score and ODI index of both groups decreased compared with that before operation,and the VSA score and ODI index of observation group were significantly lower than those of control group(P<0.05).After operation,the quality of life score of both groups was improved,and the quality of life score of the observation group was significantly higher than that of the control group(P<0.05).Six months after operation,the incidence of adjacent segment degeneration in the observation group was significantly lower than that in the control group(P<0.05).After operation,the complication rate of the observation group was significantly lower than that of the control group(P<0.05).Conclusion:Multi-level spinal fusion in the treatment of complex spinal degenerative diseases can improve the efficacy,reduce the incidence of complications,slow down the degeneration of adjacent segments,and improve spinal function.
