地佐辛复合氢吗啡酮PCEA在老年股骨骨折内固定术后镇痛中的应用效果

摘　　要：目的:研究地佐辛复合氢吗啡酮硬膜外术后自控镇痛(Post epidural patient-controlled analgesia,PCEA)在老年股骨骨折内固定术后镇痛中的应用效果。方法:选取2021年1月至2024年1月在本院进行股骨骨折内固定术的患者80例,随机将患者分为地佐辛组和氢吗啡酮组,每组各40例,地佐辛组采用地佐辛进行镇痛,氢吗啡酮组采用地佐辛复合氢吗啡酮进行镇痛,比较地佐辛组和氢吗啡酮组患者住院时间,在术后1 h(T1)、术后8 h(T2)、术后12 h(T3),比较患者视觉模拟评分(Visual Analogue Scale,VAS)、呼吸频率、平均动脉压、麻醉后不良反应发生情况。结果:两组的住院时间无显著差异。T1、T2时两组的VAS评分无显著差异,T3时氢吗啡酮组患者VAS评分低于地佐辛组(P<0.05)。T1、T3时两组呼吸频率、平均动脉压比较无显著差异,T2时地佐辛组呼吸频率低于氢吗啡酮组、平均动脉压高于氢吗啡酮组(P<0.05)。两组麻醉后不良反应发生率比较无显著差异。结论:地佐辛复合氢吗啡酮能够有效缓解患者术后疼痛,降低对患者呼吸、平均动脉压的影响。Objective:To study the analgesia effect of dezocine combined with hydromorphone for post epidural patient-controlled analgesia(PCEA)in elderly patients with femoral fracture after internal fixation.Methods:A total of 80 patients undergoing internal fixation of femoral fracture in our hospital were enrolled and randomly divided into dezocine group(dezocine for analgesia)and hydromorphone group(dezocine combined with hydromorphone for analgesia)between January 2021 and January 2024,40 cases in each group.The length of hospital stay,scores of visual analogue scale(VAS),respiratory rate and mean arterial pressure at 1 hour(T1),8 hours(T2)and 12 hours(T3)after surgery,and occurrence of adverse reactions after anesthesia were compared between the two groups.Results:There was no significant difference in length of hospital stay between the two groups.At T1 and T2,there was no significant difference in VAS scores between the two groups,VAS score in hydromorphone group was lower than that in dezocine group at T3(P<0.05).At T1 and T3,there was no significant difference in respiratory rate or mean arterial pressure between the two groups.At T2,respiratory rate in dezocine group was lower than that in hydromorphone group,while mean arterial pressure was higher than that in hydromorphone group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups after anesthesia.Conclusion:Dezocine combined with hydromorphone can effectively relieve postoperative pain and reduce effects on respiration and mean arterial pressure.
