大黄牡丹皮汤加味联合单孔腹腔镜手术治疗化脓性阑尾炎的临床分析

摘　　要：目的:探究大黄牡丹皮汤加味联合单孔腹腔镜手术治疗化脓性阑尾炎的临床疗效。方法:选取2022年1月至2024年5月期间本院收治的116例化脓性阑尾炎患者作为研究对象。采用随机数字表法将患者分为对照组与研究组,各58例。对照组接受单孔腹腔镜手术治疗;研究组在对照组基础上联合大黄牡丹皮汤加味治疗。分析对比两组的血清炎症因子水平[白细胞介素-6(Interleukin 6,IL-6)、C反应蛋白(C-reactive protein,CRP)、降钙素原(Procalcitonin,PCT)]、术后恢复情况、临床疗效及并发症发生情况。结果:两组术后12 h的IL-6、CRP、PCT水平均无明显差异(P>0.05)。术后36 h、72 h,研究组的IL-6、CRP、PCT水平均明显低于对照组(P<0.05)。研究组首次排气、肠鸣音恢复及住院时间均明显短于对照组(P<0.05)。研究组术后并发症发生率明显低于对照组(P<0.05)。两组的临床疗效无明显差异(P>0.05)。结论:化脓性阑尾炎患者联合应用单孔腹腔镜与大黄牡丹皮汤加味治疗,能减轻术后炎症,促进胃肠功能恢复,且并发症发生风险降低。Objective:To explore the clinical efficacy of Dahuang Moudan decoction combined with single-hole laparoscopic surgery in the treatment of suppurative appendicitis.Methods:A total of 116 patients with suppurative appendicitis admitted to our hospital from January 2022 to May 2024 were selected as the study objects.The patients were divided into control group and study group by random number table method,with 58 cases in each group.The control group received single-hole laparoscopic surgery;The study group was treated with Dahubarb Moudan decoction on the basis of control group.The levels of serum inflammatory factors[Interleukin-6(IL-6),C-reactive protein(CRP),Procalcitonin(PCT)],postoperative recovery,clinical efficacy and complications of the two groups were analyzed and compared.Results:There were no significant differences in IL-6,CRP and PCT levels between the two groups at 12 hours after operation(P>0.05).The levels of IL-6,CRP and PCT in the study group at 36h and 72 hours were significantly lower than those in the control group(P<0.05).The recovery of first exhaust sound,bowel sound and hospital stay in the study group were significantly shorter than those in the control group(P<0.05).The incidence of postoperative complications in the study group was significantly lower than that in the control group(P<0.05).There was no significant difference in clinical efficacy between the two groups(P>0.05).Conclusion:The combined application of single-hole laparoscopy and rhubarb Mandoupi decoction in the treatment of suppurative appendicitis can reduce postoperative inflammation,promote the recovery of gastrointestinal function and reduce the risk of complications.
