摘 要:本研究旨在对2023年7月北京市通州区发生的一起聚集性发热疫情进行病原学和基因特征研究,为相关疫情防控提供参考。本研究采用实时荧光RT-PCR对患者咽拭子标本进行肠道病毒核酸检测,并对7例肠道病毒核酸阳性分离株VP1基因进行测序和Blast型别鉴定,随后与筛选自不同国家和地区的CVA4的VP1序列的代表株进行系统发育树构建、核苷酸序列相似性及氨基酸变异位点等分析。结果显示,通州区7例肠道病毒核酸阳性标本均为柯萨奇病毒A组4型(Coxsackievirus A4,CVA4)。通过对从NCBI公共数据库筛选的来自全球的324条CVA4的全长VP1序列构建系统发育树,发现CVA4可分为A~F 6个基因型,F基因型分为F1和F2亚型。相似性分析表明,7株分离株与CVA4 F2亚型核苷酸相似性最高,中位数为0.936(0.927~0.945),与F2亚型氨基酸相似性中位数为0.993(0.990~0.996)。因此北京市通州区此次聚集性发热疫情是由CVA4引起,属于F基因型F2亚型,提示未来工作应加强对CVA4的监测。We wished to analyze the etiology and genetic characteristics of an outbreak of fever in the Tongzhou District of Beijing(China)in July 2023.We aimed to provide a reference for the prevention and control of related epidemics.Real-time reverse transcription-quantitative polymerase chain reaction was used to detect the nucleic acids(NAs)of enteroviruses(EVs)in the throat-swab specimens of patients.The VP1 gene of seven EV NA-positive isolates was sequenced,and Blast used to identify the type.A phylogenetic tree was created,and the similarity of nucleotide sequences and variations in amino-acid sites were analyzed with the representative coxsackievirus(CV)A4 VP1 sequences from different countries and regions.All seven EV NApositive samples in Tongzhou District were CVA4.A phylogenetic tree was constructed based on 324 full-length VP1 sequences of CVA4 worldwide screened from the National Center for Biotechnology Information database.CVA4 could be divided into six gene groups(A–F).The F gene group was divided into F1 and F2subgroups.Homology analyses showed that the seven isolates had the highest nucleotide similarity with the F2subgroup of CVA4;the median similarity was 0.936(0.927–0.945),and the median similarity of amino acids with the F2 subgroup was 0.993(0.990–0.996).Therefore,the fever epidemic in the Tongzhou District of Beijing was caused by CVA4,which belonged to the F2 subgroup of genogroup F.Hence,surveillance of CVA4 should be strengthened in future work.
