摘 要:本文主要研究大连市2022-2023年手足口病重要病原柯萨奇病毒A组16型的VP1区的基因进化特征。收集2022年1月-2023年12月手足口病疑似病例1677例,利用Real-time PCR方法进行肠道病毒核酸检测,利用RTPCR对CVA16核酸阳性标本VP1区进行扩增和测序,并使用肠道病毒在线定型工具(Enterovirus genotyping tool version 1.0)进行病毒型别鉴定。从GenBank下载参考序列利用DNAStar软件和MAGE11.0软件对CVA16的VP1区进行氨基酸变异分析和进化分析。结果显示全部1677个病例中,检出肠道病毒阳性1472病例,阳性率为87.78%,其中CVA16型共252个病例(17.12%)。CVA16阳性病例主要分布在3~6岁,4岁以上儿童CVA16病毒感染比例随年龄增长而呈下降趋势。大连市手足口病的2022年和2023年主要病原体分别是CVA16和CVA6,其中CVA16在2022年的主要基因型是B1c亚型,2023年的主要基因型是B1a亚型。本研究获得CVA16的VP1区序列168条,其中有82株属于B1a亚型(82/168,48.81%);73株属于B1c亚型(73/168,43.45%);13株属于B1b亚型(13/168,7.74%)。This study primarily investigates the genetic characteristics of the VP1 region of Coxsackievirus A16in hand,foot,and mouth disease(HFMD)in Dalian,China from 2022 to 2023.A total of 1,677 suspected cases of HFMD from January 2022 to December 2023 were collected.Real-time qPCR was employed to detect enterovirus nucleic acids,and the VP1 region of CVA16 positive samples was amplified and sequenced using RT-PCR.Virus typing was then conducted using the Enterovirus Genotyping Tool Version 1.0.Reference sequences were downloaded from GenBank,and amino acid mutation sites as well as genetic evolution were analyzed using DNAStar and MAGE11.0 software.The results indicated that out of the 1,677 suspected cases,1,472 were positive for enterovirus,yielding a positivity rate of 87.78%.Among these,252 cases(17.12%)were identified as CVA16 genotype.CVA16 positive cases were predominantly found in children aged 3-6 years old,with the proportion of CVA16 infections in children over 4 years decreasing with age.The primary pathogens of HFMD in Dalian,China in 2022 and 2023 were CVA16 and CVA6,respectively.The predominant CVA16 genotype in 2022 was the B1c subtype,while in 2023,it was the B1a subtype.A total of 168 VP1 sequences of CVA16 were obtained,with 82 strains classified as the B1a subtype(82/168,48.81%),73 strains as the B1c subtype(73/168,43.45%),and 13 strains as the B1b subtype(13/168,7.74%).
