摘 要:目的探究血清硫氧还蛋白1(Trx1)、白细胞衍生趋化因子2(LECT2)水平与妊娠糖尿病(GDM)患者胰岛素抵抗和胰岛细胞功能的关系。方法前瞻性选取2020年7月至2023年7月南阳市第一人民医院收治的108例GDM孕妇作为观察组,另选取同期108例孕检健康孕妇作为对照组,在孕24~28周采用酶联免疫吸附法(ELISA)检测两组孕妇的血清Trx1、LECT2水平。比较两组孕妇的一般资料、血清Trx1、LECT2、胰岛素抵抗及胰岛细胞功能的相关指标;采用Pearson法分析血清Trx1、LECT2与胰岛素抵抗和胰岛细胞功能相关指标的相关性;采用Logistic回归分析GDM的影响因素。结果观察组和对照组孕妇的空腹血糖(FPG)[(6.67±1.02)mmol/L vs(4.25±0.61)mmol/L]、30 min血糖(BG)[(9.73±2.10)mmol/L vs(7.23±1.64)mmol/L]、1 h BG[(11.48±2.75)mmol/L vs(7.41±1.34)mmol/L]、2 h BG[(10.72±2.49)mmol/L vs(6.43±1.73)mmol/L]、糖化血红蛋白(HbA1c)[6.96±1.02 vs4.86±0.54]、空腹胰岛素(FINS)[(14.57±2.67)μU/mL vs(8.24±1.53)μU/mL]、LECT2[(29.02±10.64)mg/L vs(25.18±7.59)mg/L]、胰岛素抵抗指数(HOMA-IR)[(4.13±1.15)vs(2.04±0.61)]比较,观察组明显高于对照组,差异均有统计学意义(P<0.05);1 h INS为(80.01±20.48)μU/mL、2 h INS为(60.58±15.49)μU/m L、Trx1为(2.24±0.43)ng/m L、胰岛β细胞功能指数(HOMA-β)为115.67±30.43、胰岛素曲线下面积(INSAUC)/血糖曲线下面积(PGAUC)为8.02±1.34,早期胰岛素分泌指数(△I30/△G30)为14.32±3.87,明显低于对照组的(93.54±24.57)μU/m L、(86.34±21.98)μU/mL、(4.75±1.02)ng/m L、170.85±32.14、10.76±2.59、22.15±4.87,差异均有统计学意义(P<0.05)。Pearson相关性分析结果显示,血清Trx1水平与LECT2、HOMA-IR呈负相关(P<0.05),与HOMA-β、INSAUC/PGAUC以及△I30/△G30呈正相关(P<0.05);血清LECT2水平与HOMA-IR呈正相关(P<0.05),与HOMA-β、INSAUC/PGAUC以及△I30/△G30呈负相关(P<0.05)。Logistic回归分析结果显示,FPG、HbA1c、FINS、LECT2和Trx1均是影响GDM�Objective To investigate the relationship between serum thioredoxin 1(Trx1)and leukocyte-derived chemokine 2(LECT2)levels and insulin resistance and islet cell function in patients with gestational diabetes mellitus(GDM).Methods A total of 108 pregnant women with GDM admitted to Nanyang First People's Hospital from Ju-ly 2020 to July 2023 were selected prospectively as the observation group,and another 108 healthy pregnant women in the same period were selected as the control group.Serum Trx1 and LECT2 levels of pregnant women in the two groups were detected at 24-28 weeks of pregnancy using enzyme-linked immunosorbent assay(ELISA).The general information,serum Trx1,LECT2,insulin resistance,and islet cell function related indexes were compared between the two groups;the correlation of serum Trx1,LECT2 with islet resistance and islet cell function related indexes was analyzed by Pearson's method;and logistic regression was used to analyze the influencing factors of GDM.Results Fasting blood glucose(FPG),30 min blood glucose(BG),1 h BG,2 h BG,glycated haemoglobin(HbA1c),fasting insulin(FINS),LECT2,and insulin resistance index(HOMA-IR)were(6.67±1.02)mmol/L,(9.73±2.10)mmol/L,(11.48±2.75)mmol/L,(10.72±2.49)mmol/L,6.96±1.02,(14.57±2.67)μU/mL,(29.02±10.64)mg/L,and 4.13±1.15 in the observation group,respectively,significantly higher than(4.25±0.61)mmol/L,(7.23±1.64)mmol/L,and(7.41±1.34)mmol/L,(6.43±1.73)mmol/L,4.86±0.54,(8.24±1.53)μU/mL,(25.18±7.59)mg/L,and 2.04±0.61 in the control group(P<0.05);30 min INS,1 h INS,2 h INS,Trx1,pancreatic isletβ-cell function index(HOMA-β),insulin area under the curve(IN-SAUC)/glucose area under the curve(PGAUC),and early insulin secretion index(ΔI30/ΔG30)were(80.01±20.48)μU/mL,(60.58±15.49)μU/mL,(2.24±0.43)ng/mL,115.67±30.43,8.02±1.34,and 14.32±3.87,significantly lower than(93.54±24.57)μU/mL,(86.34±21.98)μU/mL,(4.75±1.02)ng/mL,170.85±32.14,10.76±2.59,22.15±4.87 in the control group;the differences were statistically significant(P<0.05).Pearson correl
