摘　　要：目的:比较COOK球囊促宫颈成熟后早期或延迟人工破膜的引产效果。方法:回顾性分析佛山市南海区第五人民医院2022年2月—2024年4月收治的采用COOK球囊促宫颈成熟及人工破膜引产的66例足月且均为单胎妊娠的待产孕妇的临床资料,将在宫颈成熟早期进行人工破膜的待产孕妇纳入早期组(n=32),在宫颈成熟后延迟进行人工破膜的待产孕妇纳入延迟组(n=34)。比较两组分娩情况及新生儿情况。结果:延迟组阴道分娩率高于早期组,延迟组分娩后出血量少于早期组,产后不良情况发生率低于早期组,差异有统计学意义(P<0.05)。延迟组新生儿出生后1、5minApgar评分均高于早期组,差异有统计学意义(P<0.05);延迟组新生儿并发症发生率低于早期组,差异有统计学意义(P=0.037)。结论:COOK球囊促宫颈成熟后延迟人工破膜有助于提高足月待产孕妇的自然分娩率,改善产妇及新生儿情况。Objective:To compare the effects of early or delayed artificial rupture of membranes after COOK balloon induced labor.Methods:The clinical data of 66 pregnant women with full-term singleton pregnancy who were treated with COOK balloon induced labor and artificial rupture of membranes in the Foshan Nanhai District Fifth People's Hospital from February 2022 to April 2024 were retrospectively analyzed.Pregnant women who underwent artificial rupture of membranes in the early stage of cervical ripening were included in the early group(n=32),and pregnant women who delayed artificial rupture of membranes after cervical ripening were included in the delayed group(n=34).The delivery and neonatal conditions of the two groups were compared.Results:The vaginal delivery rate in the delayed group was higher than that in the early group,the amount of postpartum bleeding in the delayed group was less than that in the early group,and the incidence of adverse postpartum conditions was lower than that in the early group,and the difference was statistically significant(P<0.05).The Apgar scores of the delayed group were higher than those of the early group at 1 and 5 minutes after birth,and the difference was statistically significant(P<0.05).The incidence of neonatal complications in the delayed group was lower than that in the early group,and the difference was statistically significant(P=0.037).Conclusion:Delayed artificial rupture of membranes after COOK balloon induced cervical ripening can help increase the natural delivery rate of pregnant women at term and improve the condition of the mother and the newborn.
