全膝关节置换术治疗老年膝关节骨关节炎的临床效果分析

摘　　要：目的:研究全膝关节置换术治疗老年膝关节骨关节炎的效果。方法:选取2022年5月—2023年5月贵阳林东医院收治的100例膝关节骨关节炎老年患者为研究对象,根据随机数字表法分为对照组和观察组,各50例。对照组采用关节镜下膝关节清理术治疗,观察组采用全膝关节置换术治疗。比较两组临床疗效、膝关节功能[美国特种外科医院(HSS)量表、西安大略麦克马斯特大学骨关节炎指数量表(WOMAC)、视觉模拟评分法(VAS)评分及膝关节活动度(ROM)]、手术并发症情况。结果:观察组治疗总有效率高于对照组(P=0.037);术后1个月,两组HSS评分、ROM升高,WOMAC和VAS评分降低,观察组HSS评分和ROM高于对照组,WOMAC和VAS评分低于对照组(P<0.05);观察组手术并发症发生率较对照组低(P=0.026)。结论:与关节镜下膝关节清理术相比,全膝关节置换术治疗老年膝关节骨关节炎的效果较好,能够提高患者的膝关节功能,缓解临床症状,降低手术并发症发生率。Objective:To study the effect of total knee arthroplasty in the treatment of elderly knee osteoarthritis.Methods:A total of one hundred elderly patients with knee joint osteoarthritis admitted to Guiyang Lindong Hospital from May 2022 to May 2023 were selected as the study objects,and were divided into control group and observation group according to random number table method,with fifty cases in each group.The control group was treated with arthroscopic knee joint debridement,and the observation group was treated with total knee joint arthroplasty.The clinical efficacy,knee function[Hospital for Special Surgery(HSS)Scale,Western Ontario and McMaster University Osteoarthritis Index(WOMAC),Visual Analogue Scale(VAS)score and knee range of motion(ROM)]and surgical complications were compared between the two groups.Results:The total effective rate of observation group was higher than that of control group(P=0.037).One month after surgery,HSS score and ROM were increased,WOMAC and VAS scores were decreased,HSS score and ROM in observation group were higher than control group,WOMAC and VAS scores were lower than control group(P<0.05).The incidence of surgical complications in observation group was lower than that in control group(P=0.026).Conclusion:Compared with arthroscopic knee joint debridement,total knee arthroplasty has a better effect in the treatment of elderly knee osteoarthritis,which can improve the knee function of patients,alleviate clinical symptoms and reduce the incidence of surgical complications.
