突发性耳聋听力损失患者血脂代谢、血液流变学指标、血清缺氧诱导因子-1α和血管内皮生长因子水平变化

摘　　要：目的:观察突发性耳聋(SSNHL)听力损失患者脂代谢异常、血液流变学指标及血清缺氧诱导因子-1α(HIF-1α)、血管内皮生长因子(VEGF)水平变化。方法:选取103例SSNHL患者作为耳聋组,选取同期门诊体检听力正常志愿者105例作为对照组,对比两组患者血脂、血液流变学及血清HIF-1α、VEGF水平,并按照听力损失程度[听力损失程度<50dB(61例),听力损失≥50dB(42例)]对耳聋组进行分层分析。结果:耳聋组患者的血清甘油三酯(TG)、总胆固醇(TC)、全血黏度、血浆黏度、血清HIF-1α、VEGF水平均高于对照组(P<0.01)。两组血清高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)水平差异无统计学意义(P>0.05)。听力损失≥50dB的SSNHL患者血清TG、TC、全血黏度、血浆黏度、HIF-1α、VEGF水平均高于听力损失程度<50dB的SSNHL患者(P<0.01)。结论:SSNHL患者脂代谢和血液流变学指标异常,血清HIF-1α、VEGF水平及血液黏度升高与患者的听力损失严重程度有关。Objective:To observe the changes in lipid metabolism abnormalities,hemorheological indices,and the levels of serum hypoxia-inducible factor-1α(HIF-1α)and vascular endothelial growth factor(VEGF)in patients with sudden sensorineural hearing loss(SSNHL).Method:A total of 103 patients with SSNHL were selected as the deafness group,and 105 volunteers with normal hearing who underwent outpatient physical examinations during the same period were selected as the control group.The blood lipid,hemorheological parameters,and the levels of serum HIF-1αand VEGF were compared between the two groups.The deafness group was further analyzed by stratification according to the degree of hearing loss[degree of hearing loss<50 dB(61 cases),degree of hearing loss≥50dB(42 cases)].Results:The levels of serum triglyceride(TG),total cholesterol(TC),whole blood viscosity,plasma viscosity,serum HIF-1α,and VEGF in the deafness group were higher than those in the control group(P<0.01).There was no significant difference in the levels of serum high-density lipoprotein cholesterol(HDL-C)and low-density lipoprotein cholesterol(LDL-C)between the two groups(P>0.05).The levels of serum TG,TC,whole blood viscosity,plasma viscosity,HIF-1α,and VEGF in SSNHL patients with a hearing loss of≥50 dB were higher than those in SSNHL patients with a hearing loss of<50dB(P<0.01).Conclusion:In SSNHL patients,there are abnormalities in lipid metabolism and hemorheological indices.The increase in the levels of serum HIF-1α,VEGF,and blood viscosity is related to the severity of hearing loss in patients.
