摘 要:目的:分析妊娠期糖尿病(GDM)患者血清C1q/肿瘤坏死因子相关蛋白9(CTRP9)、微小RNA-15a(miR-15a)、miR-210水平与产后出血的相关性。方法:选取2021-02—2024-02本院就诊的GDM患者124例,依据产后出血情况分为出血组(n=33)和对照组(n=91)。比较两组患者血清CTRP9、miR-15a、miR-210水平。采用多因素Logistic回归分析GDM患者产后出血的影响因素。绘制ROC曲线分析血清CTRP9、miR-15a、miR-210对GDM患者产后出血的预测价值。结果:出血组血清CTRP9水平较对照组低,miR-15a、miR-210表达较对照组高(P<0.05)。多因素Logistic回归分析显示,miR-15a、miR-210为GDM患者产后出血的危险因素,CTRP9为保护因素(P<0.05)。受试者工作特征(ROC)曲线分析表明,血清CTRP9、miR-15a、miR-210预测GDM患者产后出血的曲线下面积(AUC)分别为0.891、0.891和0.885,三者联合的AUC为0.991,优于各自单独预测(P均<0.05)。结论:GDM产后出血患者血清CTRP9水平降低,miR-15a、miR-210表达升高,三者联合检测对GDM患者产后出血具有良好的预测价值。Objective:To investigate the relationship between the levels of serum C1q/tumor necrosis factor related protein 9(CTRP9),microRNA-15a(miR-15a),miR-210 with postpartum bleeding in women with gestational diabetes mellitus(GDM).Method:124 patients with GDM from February 2021 to February 2024 were selected and divided into the haemorrhage group(n=33)and the control group(n=91)on the basis of postpartum haemorrhage.Serum CTRP9,miR-15a and miR-210 levels were compared between the two groups.Multifactorial logistic regression was used to analyse the influencing factors of postpartum haemorrhage in GDM patients.ROC curves were plotted to analyze the predictive value of serum CTRP9,miR-15a,and miR-210 for postpartum bleeding in GDM patients.Results:The serum CTRP9 level in haemorrhage group was lower than that in the control group,while the expression of miR-15a and miR-210 was higher than that in the control group(P<0.05).Multifactorial logistic regression analysis showed that miR-15a and miR-210 were risk factors for postpartum haemorrhage in patients with GDM,and CTRP9 was a protective factor(P<0.05).ROC curve analysis showed that the area under the curve(AUC)of serum CTRP9,miR-15a,and miR-210 for the prediction of postpartum haemorrhage in patients with GDM was 0.891,0.891,and 0.885,and the AUC of the combination of the three was 0.991,which was better than their respective predictions(all P<0.05).Conclusion:Serum CTRP9 level was decreased and miR-15a and miR-210 expression were significantly increased in patients with postpartum haemorrhage of GDM,and the combined detection of the three had a good predictive value for postpartum haemorrhage in patients with GDM.
