针刺治疗神经根型颈椎病相关机制的研究进展

摘　　要：随着现代生活节奏的加快以及各种电子产品的普及,颈椎病的发病人群逐渐趋于年轻化。神经根型颈椎病(cervical spondylotic radiculopathy,CSR)是常见颈椎病类型之一,其临床发病率占颈椎病发病率的7成以上。CSR主要以颈部局部僵痛,相应神经节段支配区域出现放射性麻痛为临床表现;目前现有研究证明针刺可有效缓解CSR的临床症状,然而CSR的针刺治疗机制尚无明确阐述。本文通过查阅相关文献,整理及归纳其研究成果以探讨针刺治疗CSR的相关机制。经总结与分析,本文将从3个方面阐述针刺治疗CSR相关可能性机制的研究进展:针刺治疗CSR相关发病机制;针刺治疗CSR相关中医机制;针刺治疗CSR相关可能机制。目前现有机制证明针刺治疗CSR主要与血清炎症细胞因子浓度、肥大细胞、小胶质细胞、脊髓背角神经元等因素可能相关;虽然现有机制研究相互关联,但仍存在不足之处;现有机制研究多集中在疼痛信号上行与下行通路中单一靶点的研究,尚未形成系统化研究。同时结合针刺治疗CSR临床特点,对未来CSR相关机制研究提出思考与展望。With the acceleration of the pace of modern life and the popularization of various electronic products,the onset of cervical spondylosis becomes younger.Cervical spondylotic radiculopathy(CSR)is one of the common types of cervical spondylosis,accounting for over 70%.CSR is mainly manifested as local neck stiffness and pain,with radiation numbness and pain in the area innervated by the corresponding nerve segments.Current studies have shown that acupuncture can effectively relieve the clinical symptoms of CSR,but the therapeutic mechanism has not been clearly elucidated.This article reviews the relevant literatures,collates and summarizes the research results to explore the related mechanism of acupuncture in the treatment of CSR.Through literature review,we describe the mechanisms of acupuncture therapy for CSR through the three aspects,including the pathogenesis of acupuncture in the treatment of CSR;relevant traditional Chinese medicine(TCM)mechanism of acupuncture in the treatment of CSR;and the possible mechanisms of acupuncture in the treatment of CSR.At present,the existing mechanisms of acupuncture in the treatment of CSR are mainly related to serum inflammatory cytokine concentration,mast cells,microglia,spinal dorsal horn neurons and other factors.Although the existing mechanisms are related to each other,there are still shortcomings.The existing mechanism studies mostly focus on the study of a single target in the ascending and descending pathways of pain signals,and a systematic study is scant.Combined with the clinical characteristics of acupuncture in the treatment of CSR,we propose thoughts and prospects for the future research on the related mechanism of CSR.
