Chevron+Akin双截骨治疗重度拇外翻疗效分析

摘　　要：目的评估Chevron+Akin双截骨治疗重度拇外翻的疗效。方法回顾性分析2020年3月至2023年3月中国人民解放军联勤保障部队第九六〇医院骨科以Chevron+Akin双截骨治疗重度拇外翻共35例(46足)患者资料。所有患者均拍摄术前负重位X线、术后1周内非负重X线和术后6个月以上随访足负重位X线,测量拇外翻角,第1、2跖骨间夹角,并根据美国足踝外科学会前足评分量表(AOFAS)评价该术式的临床疗效,并记录术后截骨愈合情况及并发症。结果患者均获得随访,随访时间6~23个月,平均(12.76±4.73)个月。除2足切口延迟愈合外,其余44足切口均一期愈合。未出现骨不连情况,截骨处均愈合,愈合时间8~13周。末次随访时,拇外翻角,第1、2跖骨间夹角等影像学指标及AOFAS前足评分均优于术前,差异有统计学意义(P<0.05)。结论采用Chevron+Akin双截骨治疗重度拇外翻,效果显著,手术切口小、骨质愈合快,患者满意度高,矫正效果好。Objective To evaluate the efficacy of Chevron+Akin double osteotomy in the treatment of severe hallux valgus.Methods A total of 35 cases(46 feet)of severe hallux valgus treated with Chevron+Akin double osteotomy in the 960th Hospital of the PLA Joint Logistics Support Force from March 2020 to March 2023 were reviewed.All patients underwent preoperative weight-bearing X-ray,non-weight-bearing X-ray within 1 week after surgery,and weight-bearing X-ray of the foot followed up for more than 6 months after surgery to measure the hallux valgus angle and the angle between the 1st and 2nd metatarsals.The clinical efficacy of this procedure was evaluated according to the American Orthopedics Foot and Ankle Society Forefoot Rating Scale(AOFAS),and the postoperative osteotomy healing and complications were recorded.Results All patients were followed up for a period of 6-23 months,with an average of(12.76±4.73)months.With the exception of delayed healing of 2 osteotomies,all 44 osteotomies healed in one stage.There was no occurrence of bone non-union,and the osteotomy site healed within 8-13 weeks.At the last follow-up,all imaging indicators and AOFAS forefoot scores were better than preoperative scores(P<0.05).Conclusion The use of the Chevron+Akin double osteotomy for the treatment of severe hallux valgus has shown significant results,with small surgical incisions,fast bone healing,high patient satisfaction,and good corrective effects.
