经阴道与经腹彩色多普勒超声对子宫内膜病变诊断效能及图像质量的比较分析

摘　　要：目的比较分析经阴道与经腹彩色多普勒超声对子宫内膜病变的诊断效能及图像质量。方法选取2022年1月至2024年1月期间我院收治的子宫内膜病变患者120例,均接受经阴道彩色多普勒超声、经腹彩色多普勒超声检查。以病理学检查结果作为金标准,比较分析两种检查方式诊断子宫内膜病变的价值及图像质量。结果120例子宫内膜病变患者中,经病理学诊断最终确诊恶性病变80例,良性病变40例;经阴道彩色多普勒超声检查确诊恶性病变81例,良性病变39例;经腹彩色多普勒超声检查确诊恶性病变79例,良性病变41例。经阴道彩色多普勒超声检查诊断子宫内膜病变的灵敏度、特异度、准确性分别为96.25%、90.00%、94.17%,均显著高于经腹彩色多普勒超声检查的81.25%、65.00%、75.83%(χ^(2)=9.014、7.168、15.817,P<0.05)。经阴道彩色多普勒超声的图像质量优良率为94.17%,显著高于经腹彩色多普勒超声的73.33%(χ^(2)=9.014,P=0.003)。结论经阴道彩色多普勒超声对于子宫内膜病变具有较高的诊断价值,能降低误诊率、漏诊率,并显著提高图像质量。Objective To compare and analyze the diagnostic efficacy and image quality of transvaginal and transabdominal color Doppler ultrasound for endometrial lesions.Methods A total of 120 patients with endometrial lesions admitted to our hospital from January 2022 to January 2024 were selected and examined by transvaginal color Doppler ultrasound and transabdominal color Doppler ultrasound.The pathological examination results were used as the gold standard to compare and analyze the value and image quality of the two examination methods in the diagnosis of endometrial lesions.Results Among 120 patients with endometrial lesions,80 cases of malignant lesions and 40 cases of benign lesions were finally diagnosed by pathological diagnosis;81 cases of malignant lesions and 39 cases of benign lesions were diagnosed by transvaginal color Doppler ultrasound;79 cases of malignant lesions and 41 cases of benign lesions were diagnosed by transabdominal color Doppler ultrasound.The sensitivity,specificity and accuracy of transvaginal color Doppler ultrasound in the diagnosis of endometrial lesions were 96.25%,90.00%and 94.17%,respectively,significantly higher than 81.25%,65.00%and 75.83%of transabdominal color Doppler ultrasound(χ^(2)=9.014,7.168,15.817;all P<0.05).The excellent and good rate of image quality of transvaginal color Doppler ultrasound was 94.17%,significantly higher than 73.33%of transabdominal color Doppler ultrasound(χ^(2)=9.014,P=0.003).Conclusions Transvaginal color Doppler ultrasound has high diagnostic value for endometrial lesions,which can reduce the misdiagnosis rate and missed diagnosis rate,and significantly improve the image quality.
