摘  要:目的探讨智能三维超声成像系统(Smart MSP)筛查胎儿颅脑结构畸形的临床价值。方法回顾性分析2023年1月至2024年1月许昌市中心医院接诊的405例可疑胎儿颅脑结构畸形孕妇的临床资料,通过Smart MSP和常规产科超声检查胎儿颅脑结构。追踪新生儿出生后颅脑MRI检查或尸检结果,将其作为“金标准”统计胎儿颅脑结构畸形发生率;统计产前Smart MSP、常规产科超声对胎儿颅脑正中矢状面的直接显示率,比较产前Smart MSP、常规产科超声对胎儿颅脑结构畸形的诊断效能。结果405例孕妇的胎儿颅脑结构畸形发生率为5.68%。产前Smart MSP对胎儿颅脑正中矢状面的直接显示率与常规产科超声相比,差异无统计学意义(P>0.05)。产前Smart MSP和常规产科超声对胎儿颅脑结构畸形的诊断灵敏度、特异度和准确度均为100.00%(P>0.05)。产前Smart MSP对胎儿颅脑结构畸形不同类型的诊断灵敏度、特异度和准确度均高于常规产科超声,且与“金标准”的一致性高(Kappa值=0.944,P<0.001)。结论Smart MSP用于筛查胎儿颅脑结构畸形的价值较高,对颅脑正中矢状面的显示情况良好,并能提高产前诊断胎儿颅脑结构畸形的灵敏度、特异度与准确度。Objective To explore the clinical value of smart mid-sagittal planes(Smart MSP)in screening fetal brain structural malformation.Methods The clinical data of 405 pregnant women with suspected fetal brain structural malformation admitted to Xuchang Central Hospital from January 2023 to January 2024 were retrospectively analyzed.The fetal brain structure was examined by Smart MSP and con ventional obstetric ultrasound.The results of neonatal brain MRI after birth or autopsy were tracked as the"gold standard"to calculate the incidence of fetal brain structural malformations.The direct display rates of prenatal Smart MSP and conventional obstetric ultrasound on the median sagittal plane of fetal brain were statistically analyzed.The diagnostic efficacy for fetal brain struc tural malformation was compared between prenatal Smart MSP and conventional obstetric ultrasound.Results The incidence of fetal brain structural malformation in 405 pregnant women was 5.68%.No significant difference was found in the direct display rate of median sagittal plane of fetal brain between prenatal Smart MSP and conventional obstetric ultrasound(P>0.05).The diagnostic sensitivity,specificity,and accuracy of prenatal Smart MSP and conventional obstetric ultrasound for fetal brain structural malformation were all 100.00%(P>0.05).The sensitivity,specificity and accuracy of prenatal Smart MSP in the diagnosis of different types of fetal brain structural malformation were higher than those of conventional obstetric ultrasound,and the consistency with the"gold standard"was high(Kappa value=0.944,P<0.001).Conclusions Smart MSP is of high value in screening fetal brain structural malformation,which can display the median sagittal plane of the brain well,and improve the sensitivity,specificity,and accuracy of prenatal diagnosis of fetal brain structural malformation.
