摘 要:目的探讨氟(^(18)F)脱氧葡萄糖(^(18)F-FDG)正电子发射计算机断层扫描仪-X线计算机辅助断层仪(PET/CT)在肺结节定性诊断中的临床应用价值。方法选取2021年1月至2022年12月在河南天佑中西医结合肿瘤医院就诊的100例肺结节患者为研究对象,均接受^(18)F-FDGPET/CT检查和CT检查。以病理组织学检查结果作为金标准,比较^(18)F-FDGPET/CT、CT诊断肺结节良恶性的结果,比较^(18)F-FDGPET/CT、CT诊断肺结节良恶性的效能,分析^(18)F-FDGPET/CT与病理组织学检查结果的一致性,比较^(18)F-FDGPET/CT检查中良恶性结节患者的常规显像SUV_(max)(SUV_(max)常规)、延迟显像SUV_(max)(SUV_(max)延迟)以及滞留指数(RI)。结果病理组织学检查结果显示,100例肺结节患者中47例为良性结节,53例为恶性结节;^(18)F-FDGPET/CT检查显示良性结节46例,恶性结节54例;CT检查显示良性结节43例,恶性结节57例。^(18)F-FDGPET/CT诊断肺结节良恶性的灵敏度及准确度均高于CT,漏诊率低于CT(均P<0.05)。^(18)F-FDGPET/CT定性诊断肺结节的Kappa值为0.812,与诊断标准的一致性较高。^(18)F-FDGPET/CT检查方式中,良性结节患者的SUV_(max)常规、SUV_(max)延迟、RI值均低于恶性结节患者(均P<0.05)。结论^(18)F-FDG PET/CT诊断肺结节良恶性的灵敏度和准确度均较高,且漏诊率较低,具有较高的临床应用价值。Objective To explore the clinical application value of fluoro(^(18)F)-deoxyglucose(^(18)F-FDG)positron emission tomography/compued tomography(PET/CT)in the qualitative diagnosis of pulmonary nodules.Methods A total of 100 patients with pulmonary nodules who were treated in He'nan Tianyou Integrated Traditional Chinese and Western Medicine Tumor Hospital from January 2021 to December 2022 were selected as the study objects,and all of them received^(18)F-FDG PET/CT examination and CT ex amination.With the results of histopathological examination as the gold stan dard,the results of^(18)F-FDG PET/CT and CT in the diagnosis of benign and malignant pulmonary nodules were compared,the eficacy of ^(18)F-FDG PET/CT and CT in the diagnosis of benign and malignant pulmonary nodules was compared,the consitency of ISF-FDG PET/CT with histopathological examination was analyzed,and the routine imaging SUV_(max)SUV_(max)rauie),delay imaging SUV_(max)(SUV_(max)da)and retention index(RI)of pa tients with be nign and ma lignant nodules by^(18)F-FDG PET/CT examina tion were com pared.Results The re sults of histopatholo gy examination showed that 47 cases of 100 patients with pulmonary nodules were diagnosed as benign nodules and 53 pa tients were diagnosed as malignant nodules;^(18)F-FDG PET/CT examination showed 46 cases of benign pulmonary nodules and 54 cases of malignant pulmonary nodules;CT examination showed 43 cases of benign pulmonary nod ules and 57 cases of malignant pulmonary nodules.The sensitivity and accuracy of^(18)F-FDG PET/CT in the diagnosis of benign and malignant pulmonary nodules were higher than those of CT,and the missed diagnosis rate was lower than that of CT(all P<0.05).The Kappa value of^(18)F-FDG PET/CT in the qualitative diagnosis of pulmonary nodules was O.812,which showed high consistency with diagnostic standard.In the^(18)F-FDG PET/CT examination,the SUVnx ouie,SUVmux dehy and RI values of patients with benign nodules were lower than those of patients with malignant nodules(all P<0.05).Conclusions^
