四维彩色多普勒超声联合唐氏筛查对胎儿生长受限的诊断意义

摘　　要：目的分析四维彩色多普勒超声联合唐氏筛查对胎儿生长受限(FGR)的诊断意义。方法回顾性分析102例疑似FGR孕妇的临床资料,均接受四维彩色多普勒超声和唐氏筛查检查。以临床综合检查结果为金标准,分析四维彩色多普勒超声、唐氏筛查单一和联合检查的诊断效能,对比阳性与阴性孕妇的胎盘血管参数和雌孕激素。结果联合检查的灵敏度、准确性分别为100.00%、97.06%,均显著高于四维彩色多普勒超声的83.15%、81.37%以及唐氏筛查的78.78%、77.45%(P<0.05)。阳性孕妇的VI、FI、VFI均显著低于阴性孕妇,AFP、β-HCG水平均显著高于阴性孕妇,μE_(3)水平显著低于阴性孕妇(P<0.05)。结论四维彩色多普勒超声联合唐氏筛查诊断FGR的灵敏度和准确性高,能准确显示FGR孕妇的胎盘血管参数和雌孕激素水平。Objective To analyze the diagnostic significance of four-dimensional color Doppler ultrasound combined with Down's syndrome screening for fetal growth restriction(FGR).Methods The clinical data of 102 pregnant women with suspected FGR were retrospectively analyzed.All cases were examined by four-dimensional color Doppler ultrasound and Down's syndrome screening.With the results of comprehensive clinical examination as the gold standard,the diagnostic efficacy of single and combined examination of four-dimensional color Doppler ultrasound and Down's syndrome screening was analyzed,and the placental vascular parameters and estrogen/progesterone were compared between positive and negative pregnant woman.Results The sensitivity and accuracy of combined examination was 100.00%and 97.06%respectively,significantly higher than 83.15%and 81.37%of four-dimensional color Doppler ultrasound,and 78.78%and 77.45%of Down's syndrome screening(P<0.05).VI,FI,and VFI of positive pregnant woman were significantly lower than those of negative pregnant woman,the levels of AFP andβ-HCG were significantly higher than those of negative pregnant woman,and theμE_(3) level was significantly lower than that of negative pregnant woman(P<0.05).Conclusions Four-dimensional color Doppler ultrasound combined with Down's syndrome screening has high sensitivity and accuracy in the diagnosis of FGR,and can accurately display the placental vascular parameters and estrogen/progesterone levels of pregnant women with FGR.
