超声心动图对乳腺癌新辅助化疗患者左心功能的评估价值

摘　　要：目的探讨超声心动图对乳腺癌新辅助化疗患者左心功能的评估价值。方法选取158例乳腺癌新辅助化疗患者,均完成持续4周期的新辅助化疗,比较各时间段的左心功能、左心房参数、达峰时间(Tp)值、达峰应变(Sp)值。结果化疗前以及化疗1周期、2周期、3周期、4周期后,患者的各项左心功能指标比较,差异均无统计学意义(P>0.05)。患者化疗1周期、2周期、3周期、4周期后的LAV_(min)、LAV_(max)、LAV_(prep)均高于化疗前(P<0.05)。患者化疗1周期、2周期、3周期、4周期后的前壁、前间隔、前侧壁、下侧壁、下壁、下间隔Tp值均高于化疗前,Sp值均低于化疗前(P<0.05)。结论超声心动图检查可掌握乳腺癌新辅助化疗患者的左心功能变化,帮助及时调整治疗方案,改善预后。Objective To explore the evaluation value of echocardiography on left heart function of breast cancer patients with neoadjuvant chemotherapy.Methods 158 patients with breast cancer who had completed neoadjuvant chemotherapy for 4 cycles were selected,and the left heart function,left atrial parameters,peak time(Tp)value,peak strain(Sp)value were compared in each time period.Results Before chemotherapy and after 1,2,3,and 4 cycles of chemotherapy,no statistically significant difference was found in patients'various left heart function indicators(P>0.05).The LAV_(min),LAV_(max),and LAV_(prep) of patients after 1,2,3,and 4 cycles of chemotherapy were all higher than those before chemotherapy(P<0.05).After 1,2,3,and 4 cycles of chemotherapy,the Tp values of the front wall,front partition,front side wall,lower side wall,lower wall and lower partition of patients were higher than those before chemotherapy,while the Sp values were lower than those before chemotherapy(P<0.05).Conclusions Echocardiography can grasp the changes of left heart function of breast cancer patients with neoadjuvant chemotherapy,help to adjust the treatment plan in time and improve the prognosis.
