心脏彩超联合颈动脉超声在冠心病诊断中的临床应用价值

摘  要:目的探讨心脏彩超(UCG)联合颈动脉超声(CVUS)诊断冠心病(CHD)的临床价值。方法选取80例疑似CHD患者,均接受UCG及CVUS检查。以CAG检查结果为金标准,分析UCG、CVUS单独及联合检查诊断CHD的价值;对比CHD与非CHD患者的UCG、CVUS相关指标。结果联合检查的灵敏度、准确度、阴性预测值均高于UCG、CVUS单独检查(P<0.05)。一致性检验显示,UCG、CVUS与CAG检查结果的一致性均尚可(Kappa值=0.679、0.461,均P<0.001),联合检查与CAG检查结果的一致性良好(Kappa值=0.875,P<0.001)。与非CHD患者相比,CHD患者的LVEDV、LVESV及IMT水平均较高,LVEF较低(P<0.05)。结论UCG联合CVUS在CHD诊断中的应用价值较高,与金标准具有较高的一致性。Objective To explore the clinical value of ultrasonic cardiogram(UCG)combined with cervical vascular ultrasound(CVUS)in the diagnosis of coronary heart disease(CHD).Methods 80 patients with suspected CHD were selected.All cases received UCG and CVUS examinations.With CAG examination results as the gold standard,the diagnostic value of single and combined examinations of UCG and CVUS for CHD was analyzed.The UCG and CVUS related indicators between patients with CHD and patients without CHD were compared.Results The sensitivity,accuracy,and negative predictive value of the combined examination were higher than those of UCG and CVUS examinations alone(P<0.05).The consistency test showed that the consistencies of UCG and CVUS with CAG examination result were acceptable(Kappa values=0.679,0.461;P<0.001,P<0.001),and the consistency of combined examination with CAG examination result was good(Kappa value=0.875,P<0.001).Compared with patients without CHD,LVEDV,LVESV and IMT level of CHD patients were higher,and LVEF was lower(P<0.05).Conclusions UCG combined with CVUS in the diagnosis of CHD has high application value,and high consistency with the gold standard.
