四维超声联合NT超声应用于产前胎儿畸形筛查的临床价值探析

摘  要:目的分析四维超声联合颈项透明层厚度(NT)超声应用于产前胎儿畸形筛查的效果。方法选取2022年1月至2024年1月于本院行产前胎儿筛查的1246名孕妇,均行四维超声检查和NT超声检查。以引产或分娩结果为“金标准”,探讨四维超声、NT超声单独和联合筛查胎儿畸形的效果。结果1246名孕妇中,“金标准”检出39例胎儿畸形,四维超声检出30例,NT超声检出29例,四维超声联合NT超声检出38例。与四维超声、NT超声单独诊断相比,四维超声联合NT超声诊断胎儿畸形的灵敏度、准确度、阴性预测值均更高(P<0.05)。Kappa检验显示,四维超声联合NT超声与“金标准”的一致性极好(Kappa值=0.960,P<0.001)。结论四维超声联合NT超声能有效检出胎儿畸形,具有较高的灵敏度、特异度。Objective To analyze the effect of four-dimensional ultrasound combined with neck transparent layer thickness(NT)ultrasound applied in prenatal fetal malformation screening.Methods 1246 pregnant women who underwent prenatal fetal screening in our hospital from January 2022 to January 2024 were selected,and all cases were examined by four-dimensional ultrasound and NT ultrasound.With the results of induced labor or childbirth as the"gold standard",the effects of single and combined screening of four-dimensional ultrasound and NT ultrasound for fetal malformation were explored.Results Among 1246 pregnant women,39 cases of fetal malformation were detected by the"gold standard",30 cases were detected by four-dimensional ultrasound,29 cases were detected by NT ultrasound,and 38 cases were detected by four-dimensional ultrasound combined with NT ultrasound.Compared with single diagnosis of four-dimensional ultrasound and NT ultrasound,four-dimensional ultrasound combined with NT ultrasound had higher sensitivity,accuracy,and negative predictive value in the diagnosis of fetal malformation(P<0.05).Kappa test showed that the consistency between four-dimensional ultrasound combined with NT ultrasound and the"gold standard"was excellent(Kappa value=0.960,P<0.001).Conclusions Fourdimensional ultrasound combined with NT ultrasound can effectively detect fetal malformation,with high sensitivity and specificity.
