后外侧小切口髋关节置换术对股骨颈骨折患者术后恢复及创伤应激的影响

摘　　要：目的:探究后外侧小切口髋关节置换术对股骨颈骨折患者术后恢复及创伤应激的影响。方法:选取2023年1月—2024年12月贵阳市第二人民医院收治的120例股骨颈骨折患者为研究对象,随机分成对照组(给予常规后外侧髋关节置换术)和观察组(行后外侧小切口髋关节置换术),各60例。比较两组恢复指标、创伤应激指标和围术期指标。结果:术后,两组血清骨钙素、骨特异性碱性磷酸酶水平及Harris髋关节评分、Berg平衡量表评分升高,血清第2代骨吸收标志物水平及视觉模拟评分法评分下降,观察组优于对照组(P<0.05)。术后1、3、5 d,两组丙二醇、皮质醇水平高于术前1 d,观察组低于对照组(P<0.05);相比对照组,观察组手术时间和住院时间更短,术中出血量更少(P<0.001)。结论:后外侧小切口髋关节置换术治疗股骨颈骨折患者可以促使其术后早期恢复,缩短住院时间,减轻术后疼痛及应激反应,提高预后效果。Objective:To explore the effects of posterior lateral small incision hip replacement on postoperative recovery and traumatic stress in patients with femoral neck fracture.Methods:A total of one hundred and twenty patients with femoral neck fracture admitted to Guiyang Second People's Hospital from January 2023 to December 2024 were selected as the study objects,and randomly divided into control group(receiving conventional posterior lateral hip replacement)and observation group(receiving posterior lateral small incision hip replacement),with sixty cases in each group.Recovery indexes,traumatic stress indexes and perioperative indexes were compared between the two groups.Results:After operation,serum levels of osteocalcin,bone-specific alkaline phosphatase,Harris hip score and Berg balance scale were increased in both groups,the level of serum bone resorption markers of the second generation and the score of visual simulation were decreased,and the observation group was better than the control group(P<0.05).On day 1,3 and 5 after operation,the levels of propylene glycol and cortisol in two groups were higher than those on day 1 before operation,and those in observation group were lower than those in control group(P<0.05).Compared with the control group,the duration of operation and hospital stay in the observation group was shorter,and the amount of intraoperative blood loss was less(P<0.001).Conclusion:The treatment of patients with femoral neck fracture with posterior lateral small incision hip replacement can promote early postoperative recovery,shorten hospital stay,reduce postoperative pain and stress reaction,and improve prognosis.
