骶髂关节注射中药复合液诊断性治疗骶髂关节疼痛的临床研究

摘　　要：目的:骶髂关节注射中药复合液诊断性治疗骶髂关节疼痛的临床效果。方法:选取2021年1月—2024年6月北京市宣武中医医院和中国中医科学院望京医院骨伤科、疼痛科收治的358例骶髂关节疼痛患者作为研究对象,以随机数字表法分为观察组(脉络宁复合液治疗)及对照组(曲安奈德混合液治疗),各179例。比较两组患者临床疗效、疼痛数字评分表(NRS)评分、Roland-Morris功能障碍调查表(RMDQ)评分、Oswestry功能障碍指数(ODI)评分及腰椎活动度。结果:两组总优良率比较,无统计学差异(P>0.05);两组治疗2、4周后NRS、RMDQ、ODI评分均低于治疗前,治疗4周后评分低于治疗2周后(P<0.05);治疗4周后,两组腰椎前伸、后屈、左旋、右旋活动度均高于治疗前(P<0.05)。结论:应用骶髂关节注射中药复合液治疗骶髂关节疼痛的疗效与激素类药物相当,可缓解患者疼痛,改善腰椎及腰背部功能,增加其腰椎活动度。Objective:To study the clinical effect of sacroiliac joint pain by injection of Chinese medicine compound liquid in the diagnosis of sacroiliac joint pain.Methods:A total of three hundred and fifty-eight patients with sacroiliac joint pain admitted to Beijing Xuanwu Hospital of Traditional Chinese Medicine and Wangjing Hospital of China Academy of Chinese Medical Sciences from January 2021 to June 2024 were selected as the study objects.The patients were divided into observation group(treatment with Mailuoning compound liquid)and control group(treatment with triamcinolone mixed solution)by random number table,one hundred and seventy-nine cases each.The clinical efficacy,pain Numerical Rating Scale(NRS)score,Roland Morris Dysfunction Questionnaire(RMDQ)score,Oswestry Disability Index(ODI)score and lumbar motion were compared between the two groups.Results:There was no significant difference in the total excellent and good rate between the two groups(P>0.05).The scores of NRS,RMDQ and ODI after 2 and 4 weeks of treatment were lower than before treatment,and the scores after 4 weeks of treatment were lower than after 2 weeks of treatment(P<0.05).After 4 weeks of treatment,lumbar extension,posterior flexion,left and right rotation motion in both groups were higher than before treatment(P<0.05).Conclusion:The therapeutic effect of sacroiliac joint injection of Chinese medicine compound liquid is equivalent to that of hormone drugs in the treatment of sacroiliac joint pain,which can relieve the pain of patients,improve the function of lumbar spine and low back,and increase the motion of lumbar spine.
