摘 要:目的 探讨人工膨肺吸痰联合体位引流技术在气管切开患者肺康复中的应用效果。方法 选择2021年7月至2023年6月苏州瑞盛康复医院收治的62例气管切开患者作为研究对象,按照随机数字表法将其分为对照组与观察组,各31例。对照组采用常规排痰及气道管理,观察组在对照组基础上加用人工膨肺吸痰联合体位引流技术。比较两组的治疗效果。结果 观察组的首次排痰量多于对照组,治疗2周后排痰总量少于对照组(P<0.05)。治疗2周后,观察组的临床肺部感染评分(CPIS)评分低于对照组(P<0.05)。治疗后,两组的动脉血氧分压(PaO_(2))、动脉血氧饱和度(SaO_(2))均高于治疗前,动脉血二氧化碳分压(PaCO_(2))均低于治疗前,且观察组优于对照组(P<0.05)。观察组的气道黏膜损伤发生率显著低于对照组(P<0.05)。结论 人工膨肺吸痰联合体位引流技术应用于气管切开患者肺康复中可有效提高排痰量,改善肺部感染,提高氧合能力,且操作简单、安全可靠,值得在临床应用与推广。Objective To explore the application effect of artificial lung expansion sputum suction combined with body position drainage technology in pulmonary rehabilitation of patients with tracheotomy.Methods A total of 62 patients with tracheotomy admitted in the Suzhou Ruisheng Rehabilitation Hospital from July 2021 to June 2023 were selected as the research objects.According to the random number table method,the patients were divided into control group and observation group,with 31 cases in each group.The control group was treated with routine sputum excretion and airway management,and the observation group was treated with artificial lung expansion sputum suction combined with body position drainage technology on the basis of the control group.The therapeutic effects of the two groups were compared.Results The first sputum volume of the observation group was more than that of the control group,and the total sputum volume after 2 weeks of treatment was less than that of the control group(P<0.05).After 2 weeks of treatment,the Clinical Pulmonary Infection Score(CPIS)score of the observation group was lower than that of the control group(P<0.05).After treatment,arterial partial pressure of oxygen(PaO_(2))and arterial blood oxygen saturation(SaO_(2))in the two groups were higher than those before treatment,and arterial partial pressure of carbon dioxide(PaCO_(2))was lower than that before treatment,and those in the observation group were better than the control group(P<0.05).The incidence of airway mucosal injury in the observation group was significantly lower than that in the control group(P<0.05).Conclusion The artificial lung expansion sputum suction combined with body position drainage technology applied in pulmonary rehabilitation of patients with tracheotomy can effectively increase the amount of sputum excretion,improve lung infection,enhance oxygenation capacity,and it is simple,safe and reliable,which is worthy of clinical application and promotion.
