摘 要:目的 探讨双歧杆菌四联活菌片治疗极低出生体重儿喂养不耐受的效果。方法 选取2021年5月至2023年5月东南大学医学院附属江阴医院新生儿重症监护病房的67例出生体重<1 500 g的喂养不耐受早产儿,根据掷硬币法将其随机分为对照组(35例,早产儿配方奶喂养)和观察组(32例,双歧杆菌四联活菌片联合早产儿配方奶喂养)。比较两组的应用效果。结果 观察组的腹胀发生率低于对照组,恢复至出生体重时间、达全肠道喂养时间、肠外营养(PN)时长及光疗时长短于对照组,差异具有统计学意义(P<0.05)。两组的新生儿坏死性小肠结肠炎(NEC)、晚发型败血症(LOS)、宫外发育迟缓及28 d内死亡发生率比较,差异无统计学意义(P>0.05)。结论 双歧杆菌四联活菌片能有效改善极低出生体重儿喂养不耐受情况,缓解腹胀症状,更快完成全肠道喂养。Objective To investigate the effect of combined Bifidobacterium,Lactobacillus,Enterococcus and Bacillus cereus tablets,live on feeding intolerance in very low birth weight infants.Methods A total of 67 premature infants with feeding intolerance whose birth weight<1500 g were selected from the neonatal intensive care unit of Jiangyin Hospital Affiliated to Medical School of Southeast University from May 2021 to May 2023.According to the coin toss method,the premature infants were randomly divided into control group(35 cases,premature infant formula feeding)and observation group(32 cases,combined Bifidobacterium,Lactobacillus,Enterococcus and Bacillus cereus tablets,live combined with premature infant formula feeding).The application effects of the two groups were compared.Results The incidence of abdominal distension in the observation group was lower than that in the control group,and the time of recovery to birth weight,the time of reaching total intestinal feeding,the time of parenteral nutrition(PN)and the time of phototherapy were shorter than those in the control group,the differences were statistically significant(P<0.05).There were no significant differences in the incidences of necrotizing enterocolitis(NEC),late-onset sepsis(LOS),extrauterine growth retardation and death within 28 d between the two groups(P>0.05).Conclusion Combined Bifidobacterium,Lactobacillus,Enterococcus and Bacillus cereus tablets,live can effectively improve feeding intolerance in very low birth weight infants,relieve abdominal distension symptoms,and complete total intestinal feeding faster.
