摘 要:目的分析妊娠期高血压孕妇肠道菌群对新生儿免疫功能的影响。方法选取2021年2月至2023年2月我院收治的168例妊娠期高血压孕妇为研究对象,根据子代新生儿免疫功能将其分为免疫功能低下组(73例)和免疫功能正常组(95例)。比较两组孕妇的肠道菌群菌落数;比较两组新生儿的免疫功能指标;分析妊娠期高血压孕妇肠道菌群与新生儿免疫功能的相关性及其预测价值。结果免疫功能低下组孕妇的乳酸杆菌、双歧杆菌数量少于免疫功能正常组,肠球菌、大肠杆菌数量多于免疫功能正常组(P<0.05)。免疫功能低下组新生儿的CD3^(+)、CD4^(+)及CD4^(+)/CD8^(+)显著低于免疫功能正常组(P<0.05)。Pearson相关性分析结果显示,乳酸杆菌、双歧杆菌与新生儿CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)呈正相关,肠球菌、大肠杆菌与新生儿CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)呈负相关(P<0.05)。受试者工作特征(ROC)曲线分析结果显示,乳酸杆菌、肠球菌、双歧杆菌、大肠杆菌联合预测新生儿免疫功能低下的曲线下面积(AUC)为0.935。结论妊娠期高血压孕妇肠道菌群与新生儿免疫功能密切相关,可作为诊断临床状况、制定预防策略的有效指标。Objective To analyze the effect of intestinal flora on neonatal immune function in pregnant women with gestational hypertension.Methods A total of 168 pregnant women with gestational hypertension admitted in our hospital from February 2021 to February 2023 were selected as the research objects.According to the immune function of offspring,the pregnant women were divided into low immune function group(73 cases)and normal immune function group(95 cases).The number of intestinal flora colonies of the two groups of pregnant women was compared;the immune function indexes of the two groups of neonate were compared;the correlation between intestinal flora and neonatal immune function in pregnant women with gestational hypertension and its predictive value were analyzed.Results The number of Lactobacilli and Bifidobacteria in the low immune function group were less than those of pregnant woman in the normal immune function group,and the number of Enterococcus and Escherichia coli were more than those in the normal immune function group(P<0.05).The levels of CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)in the low immune function group were significantly lower than those of neonate in the normal immune function group(P<0.05).Pearson correlation analysis results showed that Lactobacillus and Bifidobacterium were positively correlated with neonatal CD3^(+),CD4^(+)and CD4^(+)/CD8^(+),while Enterococcus and Escherichia coli were negatively correlated with neonatal CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)(P<0.05).The results of receiver operating characteristic(ROC)curve analysis showed that the area under curve(AUC)of Lactobacillus,Enterococcus,Bifidobacterium and Escherichia coli in predicting neonatal immune dysfunction was 0.935.Conclusion The intestinal flora of pregnant women with gestational hypertension is closely related to neonatal immune function,which can be used as an effective indicator for diagnosing clinical conditions and formulating prevention strategies.
