摘 要:目的 评价计算机辅助精子分析系统检测精子浓度的性能。方法 选取2023年3月至2024年2月于徐州医科大学附属沭阳医院生殖医学中心男科就诊的患者,采集其精液标本554份。每一份精液标本分别采用手工法(显微镜)和仪器法(计算机辅助精子分析系统)进行计数。以仪器法计数精子平均浓度为分界,将所有标本分为高浓度组(258份)和低浓度组(296份);以手工法检测的每一份精子浓度值为标准,判断仪器法检测精子浓度的准确性。结果 手工法和仪器法对精子浓度的计数结果具有较高一致性(r=0.98,P<0.01)。分别以低浓度组、高浓度组的cut-off值为分界,将标本分为低值组(127例)和高值组(166例)。仪器法检测低值组精子的平均浓度与手工法检测比较,差异具有统计学意义(t=7.24,P<0.01);仪器法检测高值组精子的平均浓度与手工法检测比较,差异具有统计学意义(t=19.18,P<0.01)。结论 计算机辅助精子分析系统检测精子浓度与手工法有较高的一致性,然而当精子浓度较高或较低时,仪器法检测结果与手工法检测结果存在较大差异。当精子浓度较高时应对标本进行稀释处理;当精子浓度较低时,应对仪器法检测结果进行手工校正。Objective To evaluate the performance of computer-assisted sperm analysis system in detecting sperm concentration.Methods From March 2023 to February 2024,554 semen samples were extracted from patients who visited the department of reproductive medicine center of the Affiliated Shuyang Hospital of Xuzhou Medical University.Each semen sample was counted by manual method(microscope)and instrument method(computer-assisted sperm analysis system).Using the average concentration of sperm counted by instrument as the dividing,all samples were divided into high concentration group(258 samples)and low concentration group(296 samples);the accuracy of the instrument method to detect sperm concentration was judged by the value of each sperm concentration detected by manual method.Results The results of sperm concentration counting by manual method and instrument method were highly consistent(r=0.98,P<0.01).The specimens were divided into low value group(127 samples)and high value group(166 samples)by the cut-off value of the low and high concentration groups.The average concentration of sperm in the low value group detected by the instrument method was significantly different from that detected by the manual method(t=7.24,P<0.01);the average concentration of sperm in the high value group detected by the instrument method was significantly different from that detected by the manual method(t=19.18,P<0.01).Conclusion The computer-assisted sperm analysis system has a good consistency with the manual method in detecting sperm concentration.However,when the sperm concentration is high or low,the results of the instrument method are greatly inconsistent with the manual method.When the sperm concentration is high,the specimen should be diluted;when the sperm concentration is low,the results of the instrument method should be manually corrected.
