摘　　要：目的分析丹心降糖通脉胶囊对糖尿病合并缺血性脑卒中患者血糖变异性和凝血功能的影响。方法将2019年1月至2021年12月收治的160例糖尿病合并缺血性脑卒中患者随机分为对照组(75例)和研究组(85例)。对照组采用常规治疗,研究组采用丹心降糖通脉胶囊+常规治疗。比较两组的血糖变异性相关指标、凝血功能、内皮功能、中医证候积分及临床疗效。结果治疗后,研究组的日间血糖平均绝对差(MODD)、变异系数(CV)、平均血糖波动幅度(MAGE)、血糖标准差(SDBG)、凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(FIB)、D-二聚体(D-D)、内皮素-1(ET-1)及一氧化氮(NO)水平优于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的视物模糊、多食易积、共济失调及语言不利积分低于治疗前,且研究组低于对照组,差异具有统计学意义(P<0.05)。研究组的治疗总有效率高于对照组,差异具有统计学意义(P<0.05)。结论丹心降糖通脉胶囊治疗糖尿病合并缺血性脑卒中患者可降低血糖变异性,改善凝血功能及内皮功能,值得在临床中进一步推广和应用。Objective To analyze the effects of Danxin Jiangtang Tongmai capsule on blood glucose variability and coagulation function in patients with diabetes mellitus complicated with ischemic stroke.Methods A total of 160 patients with diabetes mellitus complicated with ischemic stroke admitted from January 2019 to December 2021 were randomly divided into control group(75 cases)and study group(85 cases).The control group was treated with conventional therapy,and the study group was treated with Danxin Jiangtang Tongmai capsule+conventional therapy.The blood glucose variability related indexes,coagulation function,endothelial function,TCM syndrome scores and clinical efficacy were compared between the two groups.Results After treatment,the absolute mean of daily difference(MODD),coefficient of variation(CV),mean amplitude of glycemic excursions(MAGE),standard deviation of blood glucose(SDBG),prothrombin time(PT),activated partial thromboplastin time(APTT),fibrinogen(FIB),D-Dimer(D-D),endothelin-1(ET-1)and nitric oxide(NO)levels in the study group were better than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the scores of blurred vision,polyphagia,ataxia and language disadvantage in the two groups were lower than those before treatment,and those in the study group were lower than the control group,and the differences were statistically significant(P<0.05).The total effective rate of treatment in the study group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Danxin Jiangtang Tongmai capsule can reduce blood glucose variability,improve coagulation function and endothelial function in patients with diabetes mellitus complicated with ischemic stroke,which is worthy of further promotion and application in clinical practice.
