摘 要:目的:检测乳糖不耐受新生儿粪便有机酸,包括乳酸、琥珀酸、短链脂肪酸总酸、乙酸、丙酸、丁酸及异丁酸的浓度,并检测新生儿粪便pH值,探讨肠道有机酸、肠道pH值在新生儿乳糖不耐受发病机制中的作用。方法:选取2023年1—12月我院新生儿科住院或门诊存在乳糖不耐受症状的新生儿18例作为LI组,选取同期某月子中心健康新生儿16例作为对照组。收集新鲜粪便,用液相色谱法检测粪便有机酸,pH计检测粪便pH值。结果:有机酸检测:LI组乳酸、琥珀酸明显高于对照组,差异有统计学意义(P<0.05);两组间SCFA总酸、乙酸、丁酸比较无统计学差异(P>0.05);LI组异丁酸明显低于对照组,差异有统计学意义(P<0.05)。LI组乳酸、琥珀酸在2~14d日龄段高于15~28d日龄段,但差异无统计学意义(P>0.05);SCFA总酸、乙酸、丁酸及异丁酸在2~14d日龄段均低于15~28d日龄段,但差异无统计学意义(P>0.05)。对照组乳酸在2~14d日龄段高于15~28d日龄段,但差异无统计学意义(P>0.05);SCFA总酸、乙酸在2~14d日龄段低于15~28d日龄段,但差异无统计学意义(P>0.05);丁酸在2~14d日龄段高于15~28d日龄段,但差异无统计学意义(P>0.05);异丁酸在2~14d日龄段低于15~28d日龄段,差异有统计学意义(P<0.05)。结论:相较于乳糖代谢终产物SCFA,乳酸、琥珀酸等结肠代谢中间产物的增高在新生儿乳糖不耐受中起着关键作用,导致肠道渗透压增高引起腹泻,而中间代谢产物的增高可能与新生儿期的肠道菌群改变有关。Objective:To investigate the role of intestinal organic acids and intestinal pH value in the pathogenesis of neonatal lactose intolerance through detecting the concentrations of fecal organic acids,including lactic acid,succinic acid,total short-chain fatty acid,acetic acid,propionic acid,butyric acid and isobutyric acid,and detecting the pH value of fecal organic acids.Methods:18 newborns with lactose intolerance were selected as the LI group from the neonatal department of our hospital from January 2023 to December 2023,and 16 healthy newborns in a month’s maternity center were selected as the control group at the same time.The fresh feces were collected to detect the organic acids by liquid chromatography,and to detect the pH value of feces by a pH meter.Results:Organic acid detection:Lactic acid and succinic acid in LI group were significantly higher than those in control group,the difference was statistically significant(P<0.05).There was no significant difference in SCFA total acid,acetic acid and butyric acid between the two groups(P>0.05).Isobutyric acid in the LI group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05).Lactic acid and succinic acid at 2~14 days of age in the LI group were higher than those at 15~28 days of age,but the difference was not statistically significant(P>0.05).The total acid,acetic acid,butyric acid and isobutyric acid of SCFA at 2~14 days of age in the LI group were lower than those at 15~28 days of age,but the difference was not statistically significant(P>0.05).The lactic acid in the control group at 2~14 days of age was higher than that at 15~28 days of age,but the difference was not statistically significant(P>0.05).The total acid and acetic acid of SCFA at 2~14 days of age in the control group were lower than those at 15~28 days of age,but the difference was not statistically significant(P>0.05).The level of butyric acid in 2~14 days of age in the control group was higher than that in 15~28 days of age,but the
