摘 要:目的 探讨对感染性骨缺损患者应用万古霉素骨水泥联合灌洗引流的效果及其对血清炎症指标的影响。方法 选取2020年1月至2023年1月福州大学附属省立医院收治的70例感染性骨缺损患者,按照不同治疗方式分为对照组40例及观察组30例,对照组给予常规抗生素灌洗引流治疗,观察组采用万古霉素骨水泥联合灌洗引流治疗。观察两组住院及骨愈合时间、血清炎症指标、疼痛情况[VAS评分]、并发症情况。结果 与对照组比较,观察组住院时间、骨愈合时间明显更短,差异有统计学意义(P <0.05);与对照组比较,观察组治疗后的降钙素原(PCT)、红细胞沉降率(ESR)、C反应蛋白(CRP)、白细胞计数(WBC)水平明显更低,差异有统计学意义(P <0.05);与对照组比较,观察组治疗1、2个月的VAS评分明显更低,差异有统计学意义(P <0.05);两组并发症发生率对比,差异无统计学意义(P>0.05)。结论 对感染性骨缺损患者应用万古霉素骨水泥联合灌洗引流治疗,可有效缩短住院时间及骨愈合时间,减轻炎症反应,缓解疼痛并控制感染,同时降低并发症的发生。Objective Exploring the effect of using vancomycin bone cement combined with lavage and drainage on patients with infectious bone defects and its impact on serum inflammatory indicators.Methods Seventy patients with infectious bone defects admitted to Fujian Provincial Hospital were selected and included from January 2020 to January 2023.They were divided into a control group of 40 patients who received routine antibiotic irrigation and drainage treatment according to different treatment methods,and an observation group of 30 patients who received treatment with vancomycin bone cement combined with irrigation and drainage.The hospitalization time,bone healing time,serum inflammatory indicators,pain condition[VAS score],and complications were observed in both groups.Results Compared with the control group,the observation group had significantly shorter hospitalization time and bone healing time,and the difference was statistically significant(P<0.05);Compared with the control group,the observation group had significantly lower levels of procalcitonin(PCT),erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),and white blood cell count(WBC)after treatment,with statistical significance(P<0.05);Compared with the control group,the VAS scores of the observation group were significantly lower after 1 and 2 months of treatment,and the difference was statistically significant(P<0.05);There was no statistically significant difference in the incidence of complications between the two groups(P>0.05).Conclusions The use of vancomycin bone cement combined with irrigation and drainage can effectively shorten the hospitalization time and bone healing time of patients with infectious bone defects,reduce their serum inflammatory index levels,effectively alleviate pain and control infection,and reduce the occurrence of complications.
