摘　　要：目的分析右美托咪定对腹腔镜下全子宫切除术患者血流动力学指标及Th1Th2平衡的影响。方法选择2020年10月至2023年3月期间甘肃省妇幼保健院收治的80例腹腔镜下全子宫切除术患者,采用随机数字表法分为观察组与对照组,每组40例。对照组采取常规麻醉,观察组在对照组基础上加用右美托咪定。比较两组的血流动力学、麻醉相关指标、Th1、Th2因子水平。结果T1、T2、T3时,两组SBP、DBP、HR水平均升高,但观察组低于对照组,P<0.05。两组拔管时间、麻醉恢复室内停留时间比较,观察组均短于对照组,P<0.05。麻醉后,两组IL-10、Th2水平降低,且观察组低于对照组,两组IL-2、Th1、Th1/Th2水平均升高,且观察组高于对照组,P<0.05。结论腹腔镜下全子宫切除术患者在实施手术麻醉时采用右美托咪定,更有助于维持血流动力学稳定,提高麻醉效果,调节Th1、Th2平衡。Objective:To analyze the effect of dexmedetomidine on hemodynamics and Th1Th2 balance in patients undergoing laparoscopic hysterectomy.Methods:80 cases of laparoscopic total hysterectomy admitted to Gansu Maternal and Child Health Hospital from October 2020 to March 2023 were randomly divided into observation group and control group,with 40 cases in each group.The control group was given routine anesthesia,and the observation group was given dexmedetomidine on the basis of the control group.Hemodynamics、anesthesia-related indexes、Th1 and Th2 levels were compared between the two groups.Results:At T1、T2 and T3,the levels of SBP、DBP and HR in both groups increased,and the observation group was lower than the control group,P<0.05.Compared with the control group,the extubation time and the indoor stay time of anesthesia recovery in the observation group were shorter,P<0.05.After anesthesia,the levels of IL-10 and Th2 in the two groups decreased,and the levels of IL-2,Th1 and Th1/Th2 in the observation group were higher than those in the control group,P<0.05.Conclusion:Dexmedetomidine is more helpful to maintain hemodynamic stability,improve anesthesia effect and adjust the balance of Th1 and Th2 in patients undergoing laparoscopic total hysterectomy.
