超声评估卵圆孔未闭相关性脑卒中的诊断及治疗研究进展

摘  要:来源不明的栓塞性卒中(embolic stroke of undetermined source,ESUS)被定义为没有实质性近端动脉狭窄或主要心脏栓塞来源的非急性脑梗死,ESUS依靠排除性诊断,通过排除已知原因来确定,这突出了在病因检查期间进行准确评估的重要性。在ESUS病因检查中,卵圆孔未闭(patent foramen ovale,PFO)更为常见,研究表明PFO可导致缺血性脑卒中,且PFO封堵术在降低60岁以下患者PFO相关卒中复发率方面优于单独的抗血小板药物。在这种情况下,超声心动图在诊断、治疗、预防PFO相关性栓塞性脑卒中方面至关重要。Embolic stroke of undetermined source(ESUS)is defined as non-acute cerebral infarction without substantial proximal arterial stenosis or major source of cardiac embolism.ESUS relies on exclusionary diagnosis and is determined by excluding known causes,which highlights the importance of accurate assessment during etiological examination.In the etiological examination of ESUS,patent foramen ovale(PFO)is more common.Studies have shown that PFO can lead to ischemic stroke,and PFO closure is superior to antiplatelet drugs alone in reducing the recurrence rate of PFO-related stroke in patients under 60 years old.In this case,echocardiography is crucial in the diagnosis,treatment,and prevention of PFO-related thromboembolic stroke.
