疾病不确定感在不孕不育患者社会支持与生育调适间的中介作用

摘　　要：目的 探讨不孕不育患者社会支持、疾病不确定感和生育调适的关系。方法 采用便利抽样法选取2023年2—4月河北省唐山市妇幼保健院生殖遗传科门诊就诊的不孕不育患者281例为研究对象,采用中文版生育调适量表评价不孕不育患者的生育调适水平、疾病不确定感量表评价不孕不育患者的疾病不确定感水平、领悟社会支持量表评价不孕不育患者的社会支持水平。应用SPSS 22.0进行统计分析,应用AMOS 24.0建立结构方程模型,采用Process 3.4.1的Bootstrap法进行中介效应检验。结果 本研究纳入281例不孕不育患者,其中男性81例,女性200例。不孕不育患者生育调适得分为(34.75±6.09)分;不孕不育患者生育调适与疾病不确定感呈正相关(P<0.01),与社会支持呈负相关(P<0.01);社会支持对不孕不育患者生育调适的直接效应为-0.46,社会支持通过疾病不确定感对不孕不育患者生育调适的间接效应为-0.18,间接效应占总效应的28.13%。结论 不孕不育患者生育调适处于中等水平,疾病不确定感在社会支持与生育调适间起部分中介作用,临床工作应重视疾病不确定感的测评,增加社会支持,以提升生育调适水平。Objective To explore the relationship between social support,disease uncertainty and fertility adjustment in infertile patients.Method The convenience sampling method was used to select 281 infertile patients in the department of Reproductive Genetics,Tangshan Maternal and Child Health Hospital from February to April in 2023.The Chinese version of Fertility Adjustment Scale,Illness Uncertainty Scale and Perceived Social Support Scale were used to evaluate the level of fertility adjustment,illness uncertainty and social support.SPSS 22.0 was used for statistical analysis,AMOS 24.0 was used to establish structural equation model,the Bootstrap method of Process 3.4.1 was used to test the mediation effect.Results This study included 281 infertile patients,81 males and 200 females.Fertility adjustment score of infertility patients was(34.75±6.09).Fertility adjustment of infertility patients was positively correlated with disease uncertainty(P<0.01),were negatively correlated with social support(P<0.01).The direct effect of social support on fertility adjustment in infertile patients was-0.46,and the indirect effect of social support on fertility adjustment of infertility patients through disease uncertainty was-0.18,accounting for 28.13%of the total effect.Conclusion The fertility adjustment of infertile patients is at a medium level,and disease uncertainty plays a partial mediating role between social support and fertility adjustment.Clinical work should pay attention to the assessment of illness uncertainty,increase social support,so as to improve the level of mental health.
