摘  要:目的探究氧化苦参碱联合奥美拉唑对重症胰腺炎患者临床中的治疗效果,分析该联合方案对患者胃肠激素水平以及肠黏膜功能的影响,以期为后续临床治疗提供参考。方法选取2022年3月—2024年3月中国人民解放军联勤保障部队第九〇八医院收治的重症胰腺炎患者140例作为研究对象,随机分为对照组和研究组,各70例。对照组采用奥美拉唑治疗;研究组采用氧化苦参碱联合奥美拉唑治疗,两组各持续治疗10d。酶联免疫吸附法(ELISA)检测淀粉酶(AMY)、胰岛素(INS)、葡萄糖(GLU)、胰蛋白酶原2(TPS2)、丙二醛(MDA)、超氧化物歧化酶(SOD)、C-反应蛋白(CRP)、淀粉样蛋白A(SAA)、白细胞介素-6(IL-6)、胃泌素(GAS)、胃动素(MTL)、血管活性肽(VIP)水平;全自动生化分析仪检测T淋巴细胞分化抗原分子4^(+)(CD4^(+))、T淋巴细胞分化抗原分子8^(+)(CD8^(+))水平,并计算CD4^(+)/CD8^(+)比值;分光光度法测定二胺氧化酶(DAO)、D-乳酸(D-LA)、肉毒素水平。比较两组患者上述胃肠激素水平和肠黏膜功能相关指标的差异。结果治疗后,研究组AMY、GLU、TPS2、CD8^(+)、MDA、CRP、SAA、IL-6、GAS、MTL、VIP、DAO、D-LA、肉毒素水平均低于对照组,差异有统计学意义(P<0.05);而研究组INS、CD4^(+)、CD4^(+)/CD8^(+)、SOD水平高于对照组,差异有统计学意义(P<0.05)。与对照组比较,研究组总有效率升高(P<0.05)。结论氧化苦参碱联合奥美拉唑治疗重症胰腺炎可以降低炎症反应、改善胃肠激素水平以及肠黏膜功能,提高免疫功能,整体治疗效果显著。Objective To explore the therapeutic effect of oxymatrine combined with omeprazole in the treatment of severe pancreatitis,and analyze the effect of this combined scheme on the gastrointestinal hormone level and intestinal mucosal function of patients,so as to provide reference for subsequent clinical treatment.Methods A total of 140 patients with severe pancreatitis admitted to Joint Support Force 908 Hospital of the Chinese People's Liberation Army from March 2022 to March 2024 were selected as the study objects.Randomly divided into control group and study group,70 cases each.The control group received omeprazole.Observation group was treated with oxymatrine combined with omeprazole.The two groups were treated continuously for 10 days.Enzyme-linked immunosorbent assay(ELISA)was used to detect amylase(AMY),insulin(INS),glucose(GLU),trypsinogen 2(TPS2),malondialdehyde(MDA),superoxide dismutase(SOD),C-reactive protein(CRP),amyloid A(SAA)and interleukin-6(IL-6).The levels of T lymphocyte differentiation antigen molecule 4^(+)(CD4^(+))and T lymphocyte differentiation antigen molecule 8^(+)(CD8^(+))were detected by automatic biochemical analyzer,and the ratio of CD4^(+)/CD8^(+)was calculated.The levels of diamine oxidase(DAO),D-lactic acid(D-LA)and Botox were determined by spectrophotometry.The differences of gastrointestinal hormone levels and intestinal mucosal function related indexes between the two groups were compared.Results After treatment,the levels of AMY,GLU,TPS2,CD8^(+),MDA,CRP,SAA,IL-6,GAS,MTL,VIP,DAO,D-LA and Botox in study group were all lower than those in control group,and the difference was statistically significant(P<0.05).The levels of INS,CD4^(+),CD4^(+)/CD8^(+)and SOD in study group were higher than those in control group,and the difference was statistically significant(P<0.05).Compared with control group,the total effective rate of study group increased(P<0.05).Conclusion The treatment of severe pancreatitis combined with omeprazole can reduce inflammation,improve gastrointestinal hormone leve
