增液汤合六磨汤加减对糖尿病合并便秘患者血清5-HT、胃动素、血管活性肠肽水平的影响

摘　　要：目的 探讨增液汤合六磨汤加减对糖尿病合并便秘患者血清胃动素(motilin, MTL)、血管活性肠肽(vasoactive intestinal peptide, VIP)、5-羟色胺(5-Hydroxytry Ptamine, 5-HT)水平的影响。方法 选择92例糖尿病合并便秘患者为研究对象,按随机数字表法分为对照组和研究组各46例。分别给予比沙可啶肠溶片治疗及增液汤合六磨汤加减治疗。观察治疗前、后两组中医证候积分、排便频率、排便时间、粪便性状、MTL、VIP、5-HT、生活质量,统计疗效。结果 治疗前,两组欲便不畅、排便时间延长、大便干结、便下艰难、食少纳呆、腹胀不适、口干、排便频率、排便时间、粪便性状分型、MTL、VIP、5-HT、心理、生理、满意度、担忧评分比较(P>0.05);治疗后,两组欲便不畅、排便时间延长、大便干结、便下艰难、食少纳呆、腹胀不适、口干、排便频率、排便时间、粪便性状分型、MTL、VIP、5-HT、心理、生理、满意度、担忧评分均较治疗前改善,与对照组相比,研究组疗效、欲便不畅、排便时间延长、大便干结、便下艰难、食少纳呆、腹胀不适、口干、排便频率、排便时间、粪便性状分型、MTL、VIP、5-HT、心理、生理、满意度、担忧评分改善更明显(P<0.05)。结论 增液汤合六磨汤加减能够有效调节糖尿病合并便秘患者血清5-HT、胃动素、血管活性肠肽水平,这可能是其治疗糖尿病合并便秘的机制。Objective This study aimed to investigate the effects of Zengye Decoction(增液汤)and Liumo Decoction(六磨汤)on the levels of serum motilin(MTL),vasoactive intestinal peptide(VIP)and serotonin(5-HT)in patients with diabetes and constipation.Methods Ninety-two patients with diabetes mellitus and constipation were selected as the research objects,and they were divided into the control group and the study group with 46 cases in each according to the random number table method.Bisacodyl enteric-coated tablets and Zengye Decoction and Liumo Decoction were given respectively.The scores of TCM syndromes,defecation frequency,defecation time,stool traits,MTL,VIP,5-HT,quality of life and statistical effects were observed before and after treatment.Results Before treatment,the poor desire to defecate,prolonged defecation time,dry stool,difficult stool,poor appetite and appetite,abdominal distension and discomfort,dry mouth,defecation frequency,defecation time,stool types,MTL,VIP,5-HT,psychology,physiology,satisfaction and worry scores of two groups had no difference(P>0.05).After treatment,the poor desire to defecate,prolonged defecation time,dry stool,difficult stool,poor appetite,abdominal distension and discomfort,dry mouth,defecation frequency,defecation time,stool types,MTL,VIP,5-HT,psychology,physiology,satisfaction and worry scores of two groups were all improved.Compared with those of the control group,the the poor desire to defecate,prolonged defecation time,dry stool,difficult stool,poor appetite,abdominal distension and discomfort,dry mouth,defecation frequency,defecation time,stool types,MTL,VIP,5-HT,psychology,physiology,satisfaction and worry scores of the study group were improved more significantly(P<0.05).Conclusion Zengye Decoction and Liumo Decoction can effectively regulate the serum levels of 5-HT,MTL and VIP in patients with diabetes complicated with constipation,which may be the mechanism of its treatment of diabetes complicated with constipation.
