金匮肾气汤加减联合复方木芙蓉涂鼻软膏治疗儿童变应性鼻炎的临床观察

摘　　要：目的 探究金匮肾气汤加减联合复方木芙蓉涂鼻软膏对变应性鼻炎(allergic rhinitis, AR)(脾肾亏虚)患儿的临床应用效果。方法 选取2021年1月—2021年12月该院就诊的AR患儿110例作为观察对象,采用随机数字表法分为观察组和对照组,每组55例。对照组给予糠酸莫米松鼻喷雾剂进行治疗,观察组在其基础上使用金匮肾气汤加减联合复方木芙蓉涂鼻软膏进行治疗。比较在治疗前、治疗1个月后两组患儿中医证候积分、鼻部客观体征(黏膜水肿、分泌物量、鼻甲肥大、黏膜苍白)评分、鼻分泌物炎症因子[白细胞介素(IL-2、IL-4、IL-10、IL-17)、嗜酸性粒细胞趋化因子(Eotaxin)]水平、生活质量[生活质量测定表(world health organization quality of life questionaire, WHOQOL-BREF)]的变化,治疗1个月后,对比在两组患儿治疗效果、不良反应发生情况。结果 治疗1个月后,观察组治疗有效率显著高于对照组(P<0.05),不良反应发生率两组比较差异无显著统计学意义(P>0.05)。治疗1个月后两组患儿中医证候积分、鼻部客观体征评分均明显降低,且观察组显著低于对照组(均P<0.05);两组患儿治疗后IL-2、IL-4、IL-17、Eotaxin水平均明显降低,且观察组显著低于对照组(P均<0.05),IL-10水平、WHOQOL-BREF评分均明显升高,观察组显著高于对照组(P均<0.05)。结论 针对AR(脾肾亏虚)患儿,金匮肾气汤加减联合复方木芙蓉涂鼻软膏的临床应用效果较好,复方木芙蓉涂鼻软膏能直接缓解患儿鼻塞症状,金匮肾气汤能助阳利水、强脾补肾从根本上治疗AR患儿,二者联用标本同治有效促进患儿康复。复方木芙蓉涂鼻软膏还能缓解糠酸莫米松鼻导致不良反应,有助于提高患儿依从性。此外考虑患儿服药依从性较差,金匮肾气汤可煎至约500 mL,分早中晚3次服用,每次约150 mL。Objective To explore the clinical application effects of Modified Jingui Shenqi Decoction(金匮肾气汤加减)combined with Compound Mufurong Nasal Ointment(复方木芙蓉涂鼻软膏)on patients with allergic rhinitis(AR)(spleen-kidney deficiency).Methods A total of 110 patients with AR who were treated in the hospital from January 2021 to December 2021 were selected as the observation subjects,and they were divided into observation group(55 cases)and control group(55 cases)by the block randomization method.The control group was treated with mometasone furoate nasal spray while the observation group was given Modified Jingui Shenqi Decoction combined with Compound Mufurong Nasal Ointment on this basis.The scores of TCM syndromes,the scores of nasal objective signs(mucosal edema,secretion volume,turbinate hypertrophy,mucosal pallor),the levels of nasal secretion inflammatory factors[interleukins(IL-2,IL-4,IL-10,IL-17)],eosinophil chemotactic factor(Eotaxin)level and quality of life[World Health Organization Quality of Life Questionnaire(WHOQOL-BREF)]before treatment and after 1 month of treatment and therapeutic effect and occurrence of adverse reactions after 1 month of treatment were compared between the two groups.Results After 1 month of treatment,the effective rate in the observation group was significantly higher than that in the control group(P<0.05),but there were no significant differences in the incidence rates of adverse reactions between the two groups(P>0.05).The scores of TCM syndromes and nasal objective signs of the two groups were significantly decreased after 1 month of treatment,and the scores in the observation group were significantly lower compared with those in the control group(all P<0.05).After treatment,the levels of IL-2,IL-4,IL-17 and Eotaxin in both groups were significantly reduced,and the levels were significantly lower in the observation group than those in the control group(all P<0.05).The level of IL-10 and the score of WHOQOL-BREF were significantly enhanced,and those in the
