桑芩止咳汤联合定喘汤加减对慢性阻塞性肺疾病急性加重期患者肺功能及血清PCT、CRP的影响

摘　　要：目的 探讨桑芩止咳汤联合定喘汤加减对慢性阻塞性肺疾病急性加重期患者肺功能及血清细胞因子的影响。方法 选取2020年11月—2021年11月该院呼吸科慢性阻塞性肺疾病急性加重期患者107例,从中筛出符合痰热郁肺证的病例80例作为研究对象,对照组40例患者予以定喘汤加减,观察组40例患者在此基础上予以桑芩止咳汤。比较两组临床疗效,中医证候积分,肺功能指标,血清细胞因子及免疫功能。结果 观察组患者总有效率(92.50%,37/40)显著高于对照组(70.00%,28/40),差异有统计学意义(P<0.05);治疗后,两组咳嗽、咯痰、胸闷、气促、喘息、胸痛评分均显著降低,差异有统计学意义(P<0.05),且观察组显著低于对照组,差异有统计学意义(P<0.05);观察组使用支气管扩张剂后1s用力呼气容积(FEV1)、残气量(residual volume, RV)、FEV1/FVC显著高于对照组,差异有统计学意义(P<0.05),观察组C反应蛋白(C-reactive protein, CRP)、降钙素原(procalcitonin, PCT)显著低于对照组,差异有统计学意义(P<0.05),观察组CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)显著高于对照组,差异有统计学意义(P<0.05),观察组CD_(8)^(+)显著低于对照组,差异有统计学意义(P<0.05)。结论 桑芩止咳汤联合定喘汤加减可以有效改善患者的中医证候评分,缓解患者的肺功能,抑制血清炎性因子水平,提高免疫能力,临床疗效较好。Objective To investigate the effect of Sangqin Zhike Decoction(桑芩止咳汤)combined with Dingchuan Decoction(定喘汤)on pulmonary function and serum cytokines in patients with acute exacerbation of chronic obstructive pulmonary disease(COPD).Methods A total of 107 patients with acute exacerbation of COPD in the respiratory Department of the hospital from November 2020 to November 2021 were selected,from which 80 patients with the syndrome of phlegm-heat stagnation of lung were screened out as the research objects.Forty patients in the control group were given Dingchuan Decoction and 40 patients in the observation group were given Sangqin Zhike Decoction on this basis.The clinical efficacy,TCM syndrome scores,pulmonary function index,serum cytokines and immune function were compared between the two groups.Results The total effective rate of the observation group(92.50%,37/40)was significantly higher than that of the control group(70.00%,28/40)(P<0.05).After treatment,the scores of cough,expectoration,chest tightness,shortness of breath,wheezing and chest pain in the two groups were significantly decreased(P<0.05),and those in the observation group were significantly lower than those in the control group(P<0.05).The values of forced expiratory volume(FEV1),residual volume(RV)and FEV1/forced vital capacity(FVC)in the observation group were significantly higher than those in the control group(P<0.05),and the levels of C-reactive protein(CRP)and procalcitonin(PCT)in the observation group were significantly lower than those in the control group(P<0.05).The levels of CD_(4)^(+) and CD_(4)^(+)/CD_(8)^(+) in the observation group were significantly higher than those in the control group(P<0.05),and the level of CD_(8)^(+) in the observation group was significantly lower than that in the control group(P<0.05).Conclusion Sangqin Zhike Decoction combined with Dingchuan Decoction can effectively improve the TCM syndrome scores,alleviate the lung function,inhibit the levels of serum inflammatory factors and improve the i
