昼夜节律在肝细胞癌中的作用机制及其中医阴阳观

摘　　要：近年来,肝细胞癌(hepatocellular carcinoma, HCC)的发病率呈现上升趋势,俨然成为了威胁人类健康的难题。许多研究表明肝脏的动态平衡需要对其代谢进行严格的昼夜节律调节,而葡萄糖、胆汁酸、血脂和胆固醇都受到定时的昼夜节律控制。表观遗传修饰和非编码RNA的转录后调控是昼夜节律的重要调节者,参与了HCC的发生发展。昼夜节律在HCC中的调控机制与中医阴阳观有着相似的内涵,在疾病早期肿瘤生长进展时,表现出腹痛、肿块、腹水等属阳症状;在疾病晚期肿瘤消耗时,出现消瘦、乏力、昏迷等属阴症状。该文以中医阴阳观探讨HCC昼夜节律的发病机理,既有助于实现中医药理论现代化,也为该病的中西医联合诊疗提供了新思路。In recent years,the incidence of hepatocellular carcinoma(HCC)has an increasing trend and has become a difficult problem threatening human health.Many studies have shown that homeostasis in the liver requires strict circadian regulation of its metabolism,while glucose,bile acids,lipids and cholesterol are all controlled by timed circadian rhythms.Epigenetic modifications and post-transcriptional regulation of non-coding RNAs are important regulators of circadian rhythms and are involved in the development of HCC.The regulatory mechanism of circadian rhythm in HCC has similar connotation to the view of Yin and Yang in TCM,and shows positive symptoms such as abdominal pain,mass and ascites when the tumor grows and progresses in the early stage of the disease;negative symptoms such as weight loss,fatigue and coma occur when the tumor consumes in the late stage of the disease.This paper discusses the pathogenesis of circadian rhythm of HCC with the view of Yin and Yang in TCM,which not only helps to realize the modernization of TCM theory,but also provides new ideas for the combined diagnosis and treatment of the disease with traditional Chinese and Western medicine.
