摘  要:目的 以糖尿病GK大鼠为研究对象,采用宏基因组学测序技术,观察肠道菌群及肠黏膜屏障的变化,探讨参芪复方对糖尿病血糖波动的影响及作用的机制。方法 建立自发性2型糖尿病血糖波动大鼠模型。实验分为空白组(NS,5 mL/kg)、模型组(NS,5 mL/kg)、参芪复方组(参芪复方浸膏,1.44 g/kg)、西格列汀组(西格列汀混悬液,16 mg/kg),除空白组外均予以高脂饲料喂养,干预8周。每周1日监测各组大鼠8∶00、10∶00、14∶00、16∶00、18∶00尾静脉血糖,计算血糖波动指标(MBG、SDBG、LAGE);检测血清LPS及结肠SIgA、ZO-1含量;观察肠道病理切片并计算组织损伤指数(TDI)评分;通过宏基因组学技术检测肠道菌群。结果 与空白组比较,参芪复方能减少GK大鼠的血糖波动幅度,降低血清LPS水平,增加肠道SIgA、ZO-1的浓度,改善肠道炎症损伤,降低TDI评分,增加拟杆菌门/厚壁菌门(B/F)比值,升高了梭杆菌门及Marinifilaceae、拟杆菌属、普氏菌属菌群丰度。结论 参芪复方能减少糖尿病血糖波动幅度,其可能与降低肠道炎症,抑制肠黏膜屏障破坏,调节肠道菌群有关,从而维持机体血糖稳态。Objective To observe the effect of Shenqi Compound(参芪复方)on blood glucose fluctuations in spontaneous type diabetic rats and explore its mechanism.Methods The spontaneous type diabetic GK rats were randomly divided into model group,Shenqi Compound group(1.44 g/kg)and sitagliptin group(16 mg/kg).The above three groups were fed with high-fat diet.Wistar rats were set as the blank group and fed with normal diet.Eighteen rats in each group were observed for 8 weeks.Then five time points(8∶00,10∶00,14∶00,16∶00,18∶00)on a fixed day every week were selected to detect the tail vein blood glucose of rats in each group,and the blood glucose fluctuation indexes[1-day mean blood glucose level(MBG),standard deviation of mean blood glucose(SDBG),maximum blood glucose fluctuation amplitude(LABG)]were determined according to the formula.The levels of serum lipopolysaccharide(LPS),secretory immunoglobulin A(SIgA)and zonula occludes protein 1(ZO-1)in colon were detected.The pathological sections of intestine were observed and the TDI scores were calculated.Finally,the intestinal flora were detected through metagenomics technology.Results Compared with the normal group,Shenqi Compound could reduce the blood glucose fluctuations,decrease the level of serum LPS,increase the concentrations of SIgA and ZO-1 in intestinal tissue,inhibit the intestinal inflammatory injury and reduce the tissue damage index(TDI)scores.It can also increase the ratio of Bacteroides to Firmicutes(B/F),and improve the abundance of Fusobacteria,Marinifilaceae,Bacteroides and Prevotella.Conclusion Shenqi Compound can inhibit intestinal inflammation,repair intestinal mucosal barrier and regulate intestinal flora,which may be the possible mechanism of reducing blood glucose fluctuation.
