妊娠期糖尿病患者血清sVCAM-1和ALP 水平与糖脂代谢紊乱的相关性研究

摘　　要：目的探讨分析妊娠期糖尿病(GDM)患者血清血管内皮细胞黏附分子-1(sVCAM-1)和碱性磷酸酶(ALP)水平与糖脂代谢紊乱的相关性。方法选择2021年6月至2022年8月山东菏泽市立医院收治的GDM患者100例,设为GDM组;另选同期在本院正常产检孕妇100例,设为正常组。检测两组孕妇血清sVCAM-1、ALP水平,分析这两项指标与糖脂代谢的相关性。结果GDM组患者sVCAM及ALP水平均显著高于正常组,P<0.05。GDM组患者TC、TG、LDL-C水平均显著高于正常组,HDL-C水平显著低于正常组,P<0.05。GDM组患者FPG、FINS、HbA1c及HOMA-IR水平均显著高于正常组,P<0.05。Pearson相关性分析显示,sVCAM与FPG、TC、TG、LDL-C、FINS、HbA1c及HOMA-IR均存在显著正相关性,与HDL-C存在显著负相关性,P<0.01;ALP与FPG、TC、TG、LDL-C、FINS、HbA1c及HOMA-IR均存在显著正相关性,与HDL-C存在显著负相关性,P<0.05。结论GDM患者血清sVCAM-1、ALP水平升高,且与糖脂代谢指标密切相关。sVCAM、ALP可作为GDM患者糖脂代谢病情变化的监测指标。Objective:To explore and analyze the correlation between the serum levels of vascular endothelial cell adhesion molecule-1(sVCAM-1)and alkaline phosphatase(ALP)and the disorder of glucose and lipid metabolism in patients with gestational diabetes mellitus(GDM).Methods:100 GDM patients admitted to Heze Municipal Hospital from June 2021 to August 2022 were selected as GDM group.Another 100 pregnant women who underwent normal check-up in our hospital during the same period were selected as the normal group.Serum levels of sVCAM-1 and ALP were detected in two groups,and the correlation between these two indexes and glucose and lipid metabolism was analyzed.Results:the levels of sVCAM and ALP in GDM group were significantly higher than those in normal group,P<0.05.The levels of TC、TG and LDL-C in GDM group were significantly higher than those in normal group,and the level of HDL-C was significantly lower than that in normal group,P<0.05.The levels of FPG、FINS、HbA1c and HOMA-IR in GDM group were significantly higher than those in normal group,P<0.05.Pearson correlation analysis showed that there was a significant positive correlation between sVCAM and FPG、TC、TG、LDL-C、FINS、HbA1c and HOMA-IR,and a significant negative correlation with HDL-C,P<0.05;ALP was positively correlated with FPG、TC、TG、LDL-C、FINS、HbA1c and HOMA-IR,and negatively correlated with HDL-C,P<0.05.Conclusion:The levels of serum sVCAM-1 and ALP in GDM patients are increased,which are closely related to the indexes of glucose and lipid metabolism.SVCAM and ALP can be used as monitoring indexes for the changes of glucose and lipid metabolism in GDM patients.
