摘 要:目的探讨显微镜联合通道下MIS-TLIF术式与经皮双通道脊柱内镜下腰椎融合术治疗单阶段腰椎间盘突出症(LDH)的临床价值。方法选择2020年1月至2022年12月惠州市第三人民医院收治的60例LDH患者,随机分为MIS-TLIF组与对照组,各30例。MIS-TLIF组采用显微镜联合通道下MIS-TLIF术式治疗,对照组采用经皮双通道脊柱内镜下腰椎融合术治疗。对比两组手术指标、疼痛程度及术后并发症。结果两组手术时间对比,P>0.05;MIS-TLIF组术中出血量、术后下床时间均少于对照组,P<0.05。两组术前、术后48 h的VAS评分对比,P>0.05;MIS-TLIF组术后4 h、12 h的VAS评分均低于对照组,P<0.05。MIS-TLIF组术后并发症发生率略低于对照组,但两组对比,P>0.05。结论显微镜联合通道下MIS-TLIF术式治疗单节段LDH具有术中出血量少、术后恢复时间快速的优点,且术后早期患者疼痛更轻。Objective:To explore the clinical value of MIS-TLIF combined with microscope and percutaneous lumbar fusion under dual-channel spinal endoscope in the treatment of single-stage lumbar disc herniation(LDH).Methods:Sixty patients with LDH admitted to the Third People's Hospital of Huizhou City from January 2020 to December 2022 were randomly divided into MIS-TLIF group and control group,with 30 patients in each group.The MIS-TLIF group was treated with MIS-TLIF under microscope combined channel,while the control group was treated with percutaneous lumbar fusion under double-channel spinal endoscope.The surgical indexes,pain degree and postoperative complications were compared between the two groups.Results:The operation time of the two groups was compared,P>0.05;The amount of bleeding during operation and the time of getting out of bed after operation in MIS-TLIF group were less than those in control group,P<0.05.The VAS scores of the two groups before and 48 h after operation were compared,P>0.05;The VAS scores of MIS-TLIF group at 4 h and 12 h after operation were lower than those of control group,P<0.05.The incidence of postoperative complications in MIS-TLIF group was slightly lower than that in control group,but the comparison between the two groups was,P>0.05.Conclusion:MIS-TLIF operation under microscope combined with channel has the advantages of less intraoperative blood loss and faster postoperative recovery time,and the pain of patients in the early stage after operation is lighter.
