摘 要:目的探索广州市成人群血清全氟化合物(PFASs)异构体水平与肺功能的关系,为采取有效措施保护心肺功能提供参考。方法于2018—2019年间,选取广州市18~89岁的成人1378人,并对其开展肺功能检查、外周静脉血采集和问卷调查。检测血清中PFASs异构体水平,包括总全氟己烷磺酸(Total-PFHx S)、直链全氟己烷磺酸(n-PFHx S)、支链全氟己烷磺酸(Br-PFHx S)和总全氟辛烷磺酸(Total-PFOS)、直链全氟辛烷磺酸(n-PFOS)、支链全氟辛烷磺酸(Br-PFOS)、全氟-1-甲基-庚烷磺酸(1m-PFOS)、全氟-6-甲基-庚烷磺酸(6m-PFOS)、全氟-3-甲基-庚烷磺酸+全氟-4-甲基庚烷磺酸+全氟-5-甲基-庚烷磺酸之和(∑3+4+5mPFOS)、全氟-2-甲基己烷之和(∑m_(2)-PFOS)。通过多重线性回归探索PFASs异构体与肺功能的关联。结果共检测1378份血样,10种血清PFASs异构体浓度均值在0.01~13.93 ng/ml之间,全氟辛烷磺酸类(PFOS)和全氟己烷磺酸类(PFHx S)浓度均是直链(n-)高于支链(Br-)异构体。调整各项混杂因素后,Br-PFOS与用力肺活量(FVC)、1 s用力呼气容积(FEV1)、3 s用力呼气量(FEV3)、每分钟最大通气量(MVV)均有负向关联,Br-PFOS每增加1个自然对数单位(ln ng/ml),分别改变-0.049 L(95%CI:-0.077~-0.021),-0.036 L(95%CI:-0.059~-0.013),-0.036 L(95%CI:-0.064~-0.009),-2.364 L/min(95%CI:-3.502~-1.226)。某些负性关联仅出现在女性人群,如Br-PFHx S、∑3+4+5m-PFOS、Br-PFOS每增加1个ln单位,仅女性FEV3减少,β分别是-0.029 L(95%CI:-0.055~-0.004),-0.022 L(95%CI:-0.038~-0.007),-0.057 L(95%CI:-0.087~-0.028)。结论血清支链异构体浓度升高与肺功能降低存在关联,且这种效应在女性人群中更为显著。Objective To understand the relationship between isomers of per-and polyfluoroalkyl substances(PFASs)and lung function in adults in Guangzhou,and to provide scientific data for making effective measures to protect cardio-pulmonary function.Methods During 2018-2019,1378 adults aged 18-89 years in Guangzhou were chosen in the present study for pulmonary function tests,and peripheral venous blood collection and questionnaire survey were conducted.Serum concentrations of PFAS isomers sum of linear and branched PFHxS(Total-PFHxS),potassium perfluorohexanesulfonate(n-PFHxS),sum of all branched isomers PFHxS(Br-PFHxS),sum of linear and branched PFOS(Total-PFOS),potassium perfluoro-1-octanesulfonate(n-PFOS),sum of all branched isomers PFOS(Br-PFOS),potassium 1-trifluoromethyperfluoroheptanesulfonate(1m-PFOS),potassium 6-trifluoromethyperfluoroheptanesulfonate(6m-PFOS),sum of all 3m-PFOS,4m-PFOS and 5m-PFOS(Σ3+4+5m-PFOS),sum of all dimethyl isomers PFOS(Σm_(2)-PFOS)were measured in the serum.Multiple linear regression was used to explore the association between isomeric PFASs and lung function.Results A total of 1378 blood samples were examined.The median values of 10 PFAS isomers ranged from 0.01 ng/ml to 13.93 ng/ml,and the concentrations of serum linear isomers(n-)were higher than branched isomers(Br-)in both PFOS and PFHxS.Adjusting for confounding factors,each one ln-unit increase in Br-PFOS decreased forced vital capacity(FVC)by 0.049 L(95%CI:0.077-0.021),forced expiratory volume in one second(FEV1)by 0.036 L(95%CI:0.059-0.013),forced expiratory volume in three seconds(FEV3)by 0.036 L(95%CI:0.064-0.009),maximum volume per minute(MVV)by 2.364 L/min(95%CI:3.502-1.226),respectively.Some negative associations were found only in females.With a one ln-unit increase on Br-PFHxS,Σ3+4+5m-PFOS and Br-PFOS,FEV3 decreased by 0.029 L(95%CI:0.055-0.004),0.022 L(95%CI:0.038-0.007),0.057 L(95%CI:0.087-0.028)in females,respectively.Conclusion The increase of serum branched isomers is associated with the decrease of lung function,
