摘 要:目的 探讨肺炎支原体(MP)感染后喘息患儿淋巴细胞亚群和细胞因子的变化情况。方法 选择2021年2月至2023年2月期间宁夏医科大学总医院收治的45例非喘息性肺炎支原体肺炎(MPP)患儿(观察组A)及45例喘息性MPP患儿(观察组B);同期选择30例健康体检儿童作为对照组。比较三组儿童T淋巴细胞(CD3+、CD4+、CD8+)、自然杀伤(NK)细胞(CD16+56+)、B淋巴细胞(CD19+)及细胞因子水平。结果 观察组组A及观察组B的CD4+、NK细胞水平均低于对照组,观察组B的CD4+、NK细胞水平均低于观察组A,P<0.05;观察组的CD8+、CD19+细胞水平均高于对照组,观察组B的CD8+、CD19+细胞水平均高于观察组A,P<0.05。观察组B儿童的IL-4、IL-17和TNF-α水平均明显高于观察组A,IL-10水平明显低于观察组A,P<0.05。结论 MPP喘息患儿存在淋巴细胞功能紊乱,具体表现为CD4+T细胞下降、CD8+T细胞上升、NK细胞下降;而且患儿的IL-4、IL-17和TNF-α水平上升,IL-10水平下降,这些因素均可导致患儿免疫功能低下。Objective:To investigate the changes of lymphocyte subsets and cytokines in wheezing children infected by Mycoplasma pneumoniae(MP).Methods:From February 2021 to February 2023,45 children with non-asthmatic mycoplasma pneumonia(MPP)and 45 children with asthmatic MPP(observation group A)were selected from the General Hospital of Ningxia Medical University.At the same time,30 healthy children were selected as the control group.The levels of T lymphocytes(CD3+、CD4+、CD8+),natural killer(NK)cells(CD16+56+)、B lymphocytes(CD19+)and cytokines were compared among the three groups.Results:The levels of BCD4+and NK cells in observation group A and observation group A were lower than those in control group,and the levels of CD4+and NK cells in observation group B were lower than those in observation group A,P<0.05.The levels of CD8+and CD19+cells in the observation group were higher than those in the control group,while those in the observation group B were higher than those in the observation group A,P<0.05.The levels of IL-4、IL-17 and TNF-αin children in observation group B were significantly higher than those in observation group A,and the level of IL-10 was significantly lower than that in observation group A,P<0.05.Conclusion:There is lymphocyte dysfunction in MPP wheezing children,which is manifested by the decrease of CD4+T cells,the increase of CD8+T cells and the decrease of NK cells.Moreover,the levels of IL-4、IL-17 and TNF-αincreased,and the level of IL-10 decreased,all of which can lead to low immune function.
