补脾益肾法治疗慢性肾功能衰竭合并血液透析贫血的临床疗效

摘　　要：目的:探讨补脾益肾法治疗慢性肾功能衰竭合并血液透析贫血的临床疗效。方法:选取2022年1月至2024年1月期间在尤溪县中医医院治疗的60例慢性肾功能衰竭合并血液透析贫血患者,采用随机表法分为对照组与观察组,每组30例。对照组患者接受常规西药治疗,观察组在对照组的基础上接受补脾益肾法治疗。比较两组患者肾功能指标[尿蛋白排泄量(UAER)、白蛋白/肌酐比值(ACR)、尿肝型脂肪酸结合蛋白(L-FABP)]、贫血指标[红细胞计数(RBC)、血红蛋白(Hb)、红细胞压积(Hct)]以及中医证候积分(腰膝酸软、食欲不振、恶心呕吐、夜尿频繁)。结果:治疗后观察组患者UAER、ACR、尿L-FABP水平比对照组低,差异具有统计学意义(P<0.05);治疗后观察组患者RBC、Hb、Hct水平比对照组高,差异具有统计学意义(P<0.05);治疗后观察组患者各项中医证候积分比对照组低,差异具有统计学意义(P<0.05)。结论:补脾益肾法可有效改善慢性肾功能衰竭合并血液透析贫血患者的肾功能,并对贫血状况有较好的改善作用,可作为慢性肾功能衰竭合并血液透析贫血患者治疗的辅助方法。Objective To explore the clinical efficacy of the method of invigorating spleen and benefiting kidney in the treatment of chronic renal failure with hemodialysis anemia.Methods A total of 60 patients with chronic renal failure and hemodialysis anemia treated at Youxi Hospital of Traditional Chinese Medicine from January 2022 to January 2024 were selected and randomly divided into a control group and an observation group,with 30 cases in each group.The control group received conventional Western medicine treatment,while the observation group received the method of invigorating spleen and benefiting kidney in addition to the treatment in the control group.The renal function indicators[urinary albumin excretion rate(UAER),albumin/creatinine ratio(ACR),urinary liver-type fatty acid-binding protein(L-FABP)],anemia indicators[red blood cell count(RBC),hemoglobin concentration(Hb),hematocrit(Hct)],and traditional Chinese medicine symptom scores(lumbar and knee weakness,loss of appetite,nausea and vomiting,frequent nocturia)were compared between the two groups.Results After treatment,the levels of UAER,ACR,and L-FABP in the observation group were lower than those in the control group,with statistically significant differences(P<0.05);the levels of RBC,Hb,and Hct in the observation group were higher than those in the control group,with statistically significant differences(P<0.05);the traditional Chinese medicine symptom scores in the observation group were lower than those in the control group,with statistically significant differences(P<0.05).Conclusion The method of invigorating spleen and benefiting kidney can effectively improve the renal function of patients with chronic renal failure undergoing hemodialysis and anemia,and has a good effect on improving anemia,which can be used as an auxiliary treatment method for such patients.
