肺部寡育外瓶霉感染1例

摘  要:目的探索寡育外瓶霉肺部感染治疗经验。方法总结潍坊医学院附属医院呼吸内科2022年5月收治的1例寡育外瓶霉肺部感染病人的诊疗方案。结果病人主诉发现右肺病变4 d。入院后给予哌拉西林他唑巴坦、左氧氟沙星抗细菌及非典型病原体治疗1周后胸部CT发现未见好转,行气管镜及气管刷检物病理脱落细胞检查、肺泡灌洗液细胞分类计数及感染病原宏基因组学检测,最终检测出寡育外瓶霉疑似,嘱其口服伊曲康唑胶囊0.1 g,每12小时1次,1个月后复查胸部CT,病变较前明显好转。结论伊曲康唑等三唑类抗真菌药物对寡育外瓶霉有很好的疗效。Objective To explore the experience of treating pulmonary infection with exophiala oligosperma in patients.Methods The diagnosis and treatment plan of a patient with pulmonary infection with exophiala oligosperma admitted to the respiratory medicine department of Affiliated Hospital of Weifang Medical University in May 2022 was summarized.Results The patient complained of right lung lesions for 4 days.After admission,he was given piperacillin-tazobactam and levofloxacin to treat bacterial and atypical pathogens for 1 week.However,chest CT showed no improvement.Therefore,bronchoscopy,bronchial brushing pathology testing,alveolar lavage fluid cell classification and counting,and infection pathogen metagenomic testing were performed.Finally,exophiala oligosperma was detected and the patient was prescribed itraconazole capsules oral dosage of 0.1 g every 12 hours.After one month,chest CT showed significant improvement compared to before.Conclusion Triazole antifungal drugs such as itraconazole have good therapeutic effects on exophiala oligosperma.
