温针灸联合平衡舒筋手法推拿对脑卒中偏瘫病人运动功能及肌力的影响

摘　　要：目的探究脑卒中偏瘫病人采用温针灸联合平衡舒筋手法推拿干预的治疗效果。方法2021年1月至2022年6月将武汉市第三医院确诊为脑卒中的90例偏瘫病人按照随机抽样法分成对照组(n=45)与联合组(n=45),对照组病人给予常规治疗结合温针灸治疗,联合组在对照组的基础上联合平衡舒筋手法推拿治疗。对比两组治疗效果、肢体运动功能(肢体运动能力、肢体平衡能力)、肌力水平、生活自理能力和生活质量情况。结果治疗后,联合组病人治疗总有效率93.33%(42/45)显著高于对照组75.56%(34/45)(P<0.05);两组肢体运动Fugl-Meyer功能评定量表(FMA)、Berg平衡量表(BBS)、英国医学研究委员会(MRC)评分均升高,联合组高于对照组(P<0.05);两组改良Barthel指数评定量表(MBI)、脑卒中专用生活质量量表(SS-QOL)评分均升高,联合组高于对照组(P<0.05)。结论温针灸联合平衡舒筋手法推拿治疗脑卒中偏瘫病人效果显著,可明显改善病人肢体运动能力和平衡能力,促进肌力水平恢复,提高生活自理能力和生活质量。Objective To explore the therapeutic effect of warming acupuncture and moxibustion combined with balanced tendon relaxing manipulation intervention on patients with stroke hemiplegia.Methods Ninety patients with hemiplegia diagnosed as stroke in Wuhan Third Hospital from January 2021 to June 2022 were randomly grouped into a control group(n=45)and a combined group(n=45).The patients in the control group were treated with warming acupuncture and moxibustion,and the combined group was treated with balanced tendon relaxing manipulation on the basis of warming acupuncture and moxibustion.The treatment effect,limb motor function[Fugl-Meyer Assessment(FMA)Scale,Berg Balance Scale(BBS)],Medical Research Council(MRC)muscle strength level,self-care ability and quality of life were compared between the two groups.Results After treatment,the total effective rate of the combined group was 93.33%(42/45),which was significantly higher than 75.56%(34/45)of the control group(P<0.05);The Fugl-Meyer assessment(FMA),Berg balance scale(BBS)and British Medical Research Council(MRC)scores of limb movement in the two groups were increased,and the scores in the combined group were higher than those in the control group(P<0.05);The modified Barthel index(MBI)and stroke specific quality of life(SS-QOL)scores of the two groups were increased,and the scores of the combined group were higher than those of the control group(P<0.05).Conclusion Warming acupuncture and moxibustion combined with balanced tendon relaxing manipulation has an obvious effect on patients with stroke hemiplegia,which can obviously improve their limb movement ability and balance ability,promote the recovery of muscle strength,and improve self-care ability and quality of life.
