健脾益气汤联合RPH对直肠黏膜内脱垂患者直肠生理功能及炎症指标的影响

摘　　要：目的:探索健脾益气汤联合自动痔疮套扎术(RPH)对直肠黏膜内脱垂患者直肠生理功能及应激指标的影响。方法:选取龙岩人民医院2022年1月至2024年1月期间收治的82例气虚下陷型直肠黏膜内脱垂患者作为研究对象,采用摸球法分为操作组和方药组,各41例。操作组患者行RPH治疗,方药组患者在操作组基础上增加健脾益气汤治疗,比较两组患者中医症状积分、临床疗效、炎症指标及直肠生理功能。结果:方药组患者术后3周肛门坠胀、排便无力、气短乏力评分较操作组更低,差异具有统计学意义(P<0.05);方药组患者治疗总有效率为90.24 %(37/41),高于操作组的70.73 %(29/41),差异具有统计学意义(P<0.05);方药组患者术后1周血清肿瘤坏死因子–α(TNF–α)、白细胞介素–6(IL–6)、C反应蛋白(CRP)水平较操作组低,差异具有统计学意义(P<0.05);方药组患者术后6个月直肠最大收缩压、直肠最大静息压水平较操作组高,直肠感觉容量水平较操作组低,差异具有统计学意义(P<0.05)。结论:健脾益气汤联合RPH可改善直肠黏膜内脱垂患者直肠生理功能,降低中医症状积分,调控炎症指标,提高临床疗效。Objective To explore the effect of Jianpi Yiqi Decoction combined with Rubber Band Ligation (RPH) on rectal physiological function and stress indices in patients with internal rectal mucosal prolapse.Methods A total of 82 patients with Qi deficiency and sinking type internal rectal mucosal prolapse were selected from Longyan People’s Hospital from January 2022 to January 2024 as the research subjects.They were divided into operation group and formula group using the ball method,with 41 cases in each group.Patients in the operation group received RPH treatment,while patients in the formula group received additional treatment with Jianpi Yiqi Decoction based on the operation group.The traditional Chinese medicine (TCM) symptom score,clinical efficacy,inflammatory factor,and rectal physiological function were compared between the two groups.Results At 3 weeks postoperatively,the scores for anal distension,difficulty in defecation,and shortness of breath in the formula group were lower than those in the operation group,with statistically significant differences (P < 0.05).The total effective rate of treatment in the formula group was 90.24% (37/41),higher than 70.73% (29/41) in the operation group,with statistically significant difference (P < 0.05).At 1 week postoperatively,the levels of serum tumor necrosis factor–α (TNF-α),interleukin–6 (IL-6),and C–reactionprotein (CRP) in the formula group were lower than those in the operation group,with statistically significant differences (P < 0.05).At 6 months postoperatively,the maximum systolic pressure and maximum resting pressure of the rectum in the formula group were higher than those in the operation group,and the rectal sensory capacity was lower than that in the operation group,with statistically significant differences (P < 0.05).Conclusion Jianpi Yiqi Decoction combined with RPH can improve the rectal physiological function of patients with internal rectal mucosal prolapse,regulate TCM symptom scores,control inflammatory factor,and enhance clinical effi
