艾滋病患者高效抗逆转录病毒治疗免疫学失败的影响因素分析

摘　　要：目的:探讨艾滋病患者高效抗逆转录病毒治疗(HAART)免疫学失败的影响因素。方法:选择2020年5月至2023年1月于赣州市第五人民医院接受HAART的98例艾滋病患者,收集研究对象的临床资料,分析艾滋病患者HAART免疫学失败的影响因素。结果:98例行HAART的艾滋病患者中,免疫学失败27例(27.55%),免疫重建成功71例(72.45%);多因素logistic回归分析显示:年龄≥60岁、注射吸毒感染、合并其他感染、确诊至开始治疗时间≥12个月、基线簇分化抗原(CD)4^(+)T淋巴细胞计数<200个·μL^(-1)、用药依从性差/中等是艾滋病患者HAART免疫学失败的危险因素(P<0.05),含整合酶抗病毒治疗方案是其保护因素(P<0.05)。结论:艾滋病患者HAART免疫学失败比例较高,其受年龄、感染途径、合并其他感染、确诊至开始治疗时间、用药方案、用药依从性、基线CD4^(+)T淋巴细胞计数影响。Objective To explore the factors influencing the immunological failure of highly active antiretroviral therapy(HAART)in acquired immunodeficiency syndrome(AIDS)patients.Methods Select 98 AIDS patients who received HAART at the Fifth People's Hospital of Ganzhou from May 2020 to January 2023,collect clinical data of the subjects,and analyze the factors affecting the immunological failure of HAART in AIDS patients.Results Among 98 AIDS patients who underwent HAART,27 cases(27.55%)had immunological failure and 71 cases(72.45%)had successful immune reconstitution;Multivariate logistic regression analysis showed that:age≥60 years old,injection drug infection,combined with other infections,time from diagnosis to initiation of treatment≥12 months,baseline cluster differentiation antigen(CD)4^(+)T lymphocyte count<200μL^(-1),poor/moderate medication compliance were the risk factors for HAART immune failure in AIDS patients(P<0.05),and antiviral therapy with integrase was the protective factor(P<0.05).Conclusion The proportion of HAART immune failure in AIDS patients is high,which is affected by age,route of infection,combined with other infections,the time from diagnosis to the start of treatment,medication regimen,medication compliance,and baseline CD4^(+)T lymphocyte count.
