摘 要:目的:探讨胸部计算机断层扫描(CT)定位像预测肺体积的可行性。方法:从2022年3月至2022年8月广州医科大学附属第一医院的图像存档与通信系统(PACS)中回顾性选取150例受检者的胸部CT图像作为试验组,另外选取55例受检者作为外部验证组。对患者胸部CT定位像的肺野范围进行勾画并测算肺野面积,后对胸部CT平扫图像进行肺体积分割并计算肺体积。分析试验组胸部CT定位像与CT肺体积的相关性,并结合试验组人口学指标建立肺野面积预测肺体积的回归方程,后结合外部验证组检查结果,分析预测的肺体积与CT肺体积的相关性。结果:试验组受检者总肺、右肺和左肺的肺野面积与CT肺体积均呈正相关(r=0.893、0.855、0.867,P<0.05)。多元线性回归结果显示,肺野面积(S)预测肺体积(V)的预测方程为:V=-586.209+13.177×S+11.898×体质量+5.106×年龄-189.279×性别(男性:1;女性:2),R2=0.837,调整后R2=0.832,P<0.001,错误率为8%。试验组和外部验证组受检者通过预测方程得到的肺体积与CT肺体积均呈正相关(r=0.915、0.804,P<0.001)。结论:胸部CT定位像的肺野面积与CT测得的肺体积相关性高,可用于预测肺体积。Objective To explore the feasibility of using chest computed tomography(CT)localization images to predict lung volume.Methods From March 2022 to August 2022,chest CT images of 150 patients were restrospectively selected from the Picture Archiving and Communication System(PACS)of the First Affiliated Hospital of Guangzhou Medical University as the experimental group,and another 55 patients were selected as the external validation group.The lung field range on the chest CT localization images was delineated and the lung field area was measured.Subsequently,lung volume segmentation was performed on the chest CT scan images to calculate lung volume.The correlation between chest CT localization images and CT lung volume in the experimental group was analyzed,and a regression equation for predicting lung volume based on lung field area was established using demographic indicators of the experimental group.The results were then validated with the external validation group,and the correlation between the predicted lung volume and CT lung volume was analyzed.Results In the experimental group,the lung field area was positively correlated with CT lung volume for the total lung,right lung,and left lung(r=0.893,0.855,0.867,respectively;P<0.05).The multiple linear regression results showed that the prediction equation for lung volume(V)based on lung field area(S)was:V=-586.209+13.177×S+11.898×body mass+5.106×age-189.279×gender(male:1;female:2),with R2=0.837,adjusted R2=0.832,P<0.001,and an error rate of 8%.The lung volumes obtained from the prediction equation in both the experimental and external validation groups were positively correlated with CT lung volumes(r=0.915,0.804,respectively;P<0.001).Conclusion The lung field area on chest CT localization images has a high correlation with the lung volume measured by CT and can be used to predict lung volume.
